SUBJECT: Z -- MINOR REPAIRS TO CONCRETE DECK ON BRIDGE SOL bridge00387 DUE 062501 POC T.C.Mitchell DEC: Supply all labor, material, supervision, equipment and transportation to perform minor repairs to concrete deck, to concrete curbs to South Farm Bridge and to seal with epoxy paint. The Contractor shall perform the work as follows: 1. clean debris from bridge deck and four corners of the bridge, 2. apply epoxy concrete to minor cracks with a slight slope so that the bridge deck to drains toward the drainage outlets, 3. apply epoxy paint on the concrete deck and the concrete curbs, 4. brush clean and remove minor rust to steel handrails and apply two coats of rustproof paint, 5. install concrete drainage ditches next to the bridge abutments at four locations, 6. remove deteriorated expansion joint seals and reseal properly. The contractor shall submit data for epoxy paint and joint caulking for approval. The bridge is located on the South Farm, Agriculture Research Service, USDA, Beltsville, Maryland, 20705. This job is to be completed 60 days after Notice to proceed by the Contracting Officer. All work shall be in compliance with the Statement of work, USDA special conditions, USDA clauses, and all other specifications and codes cited therein and wage rate MD000057 mod.2. This is a Request for Quote (RFQ). Only one site visit will be conducted. Any questions must be in writing ONLY by fax to 301/504-0333 or mail questions to address on or before June 13, 2001. No answers will be mailed prior to that date. Site visit is scheduled for June, 5, 2001, 10am at the bridge. Quotes will be accepted either by fax or by mail. Magnitude of project is between $25,000 and $100,000 and will be procured using Simplified Acquisition Procedures 100% small business set-aside. A firm fixed price purchase order will be awarded to the responsible/responsive offeror whose offer conforms to the solicitation and is considered most advantageous to the Government. NAICS code is 234120 with a small business standard of 27.5 million. RFQ will be issued/mailed on May 23, 2001. Limited amount of copies available on a first come first serve basis. Quotes are due by June 25, 2001 COB. Anticipated award date June 29, 2001. All offerors must include with their quote a completed copy of the Certifications and Representations FAR 52.204-3 Taxpayer Identification, 52.209-5 Certification Regarding Debarment Suspension and Other Responsibility Matters, and 52.219-1 Small Business Program Representation. Also, FAR clause 52.228.13 Alternative Payment Protection applies to the successful offeror. A Payment Bond or Irrevocable Letter of Credit must be presented to the Contracting Officer within 10 days after Notice to Proceed.
