Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number F41612-01-T0071 is hereby issued as a Request For Quote (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. This acquisition is under North American Industrial Code 339113, with a small business size standard of 500 employees. This acquisition is for Full and Open. Provide the following items: (1) 44 EA/FR-M40 Facepiece Weapons of Mass Destruction Chemical/Biological agent respiratory protection facepiece. (2) 88 EA/FR-C2A1 Cartridge Chemical/Biological agent filter cartridge for facepiece. (3) 44 PR/FR-M40-1 Eyepiece Outsert Protective lens cover for facepiece. (4) 13 EA/FR-M40-4 Spectacle Kit Prescription eyeglass inserts for facepiece. (5) 2 PG/FR-M40-101 Head Harness replacement harness assembly for anticipated wear and tear. (6) 2 PG/FR-M40-102 Inhalation Valve Kit replacement kit for wear and tear and maintenance. (7) 1 PG/FR-M40-103 Nosecup Valve replacement nosecup valve for wear and tear and maintenance. (8) 2 PG/FR-M40-104 Exhalation valve cover replacement cover for wear and tear and maintenance. (9) 2 PG/6889 Exhalation valve disk replacement valve disk for wear and tear and maitenance. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR 52.212-1, Instructions to Offerors_Commercial Items (Mar 2000). It is incorporated by reference and applies to this acquisition. Award will be based on price alone. Each offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certification_Commercial Items (Feb 2000) with their offer. FAR 52.212-3, Alternate I and III, must also be completed. FAR 52.212-4 (May 1999), Contract Terms and Conditions_Commercial Items, is incorporated by reference and applies to this acquisition with the addition of DFARS 252.204-7004, Required Central Contractor Registration (Mar 1998). The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders_Commercial Items (Aug 2000) applies to this acquisition and is incorporated by reference. DFARS Clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions Of Commercial Items (Mar 2000) is included by reference with the following applicable clauses.
