Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6. as supplemented with additional information included in this notice. This procurement is being conducted under the Simplified Acquisition Procedures. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation. THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Proposal is RFP-797-FDF3- 00-0137. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-25. This solicitation is issued on an unrestricted basis under NAIC 561621, with a size standard of $9.0. The Air Force Medical Logistics Office has a requirement for a Security Door Access Expansion System to complement the existing Apogee Automation Advanced Life Safety System which ties into the security system, fire alarm, HVAC and surveillance camera systems. Place of delivery is the 99th Medical Group/SGSL, 4700 Las Vegas Boulevard North, Building 1300, Nellis AFB, NV 89191-6601. Delivery shall occur within 160 calendar days after award of the purchase order. A SITE VISIT will be held on May 30, 2000 at 9:00 am at the Michael O'Callaghin Hospital, Bldg 1300, RM l-217, Nellis AFB, NV 89191-6601. Please contact Captain Michael B. Ambruso, USAF, at 702-653-2941 to sign up for the site visit. This is the only site visit to be scheduled. General: The contractor shall be responsible for the design, installation, parts, materials, equipment, labor, transportation freight charges, and training of personnel as required to install and expand the existing system and integrate a new combined DSX Total Access Control System as follows: (1) install a quantity of 106 additional magnetic stripe tape readers on 103 doors; (2) install a quantity of 28 additional magnetic locks; (3) install a quantity of 99 electric strike locks; (4) install a quantity of 17 remote release buttons for electric strike locks; (5) install a quantity of 4 key override devices; (6) install a quantity of 7 exit buttons; (7) provide new digital camera kit for photo badging; (8) add new communications networks and new electronic controls; (9) combine the assisting system with the new system; (10) engineer and design the new and combined system; (11) deliver drawing submittals for new combined system; (12) install new combined system and new combined system software; (13) program the new combined system; (14) commission and trouble shoot the new combined system; (15) provide training to hospital personnel on the new combined system; (16) DSX assess system shall tie in to the existing Windows NT server workstation located in Fire Command; (17) install a quantity of 2 ID card readers at the Information Desk for recall purposes (readers shall provide reports of all entrants); (18) install a card reader capable of reading a Department of Defense ID card at the Pharmacy Front Desk (a stand alone terminal is required); (19) install elevator control feature on elevators 2A and 2B; (20) install automatic door opening sensor on dock door and disconnect card reader from assisting automatic opener; (21) all items installed shall be connected to emergency power; (22) a quantity of 24 doors will be connected to the fire alarm system; (23) service manuals and warranties shall be provided. Contractor Personnel: The contractor shall ensure that all services under this contract are performed by competent certified personnel who have completed the applicable installation, maintenance and training courses. The contractor shall have hands on experiences, programming experience, and a working knowledge of DSX Total Access Control Systems. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror -- Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to is necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification -- Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition.
