Description

The Great Lakes Acquisition Center (GLAC ) is soliciting quotations to provide VA Hospital Tomah, WI the services of two (2) licensed Physical Therapists. There will be two periods of performance, one for each contract Physical Therapist. The period of performance for the 1st Physical Therapist will be June 18,2001 through August 17,2001. The period of performance for the second physical therapist will be July 2,2001 through September 6,2001. Cost per Hour to be all-inclusive (all labor, supervision, transportation, per diem, etc.). Contract will be firm fixed price. The NAICS number is 621340. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof.
