Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and Part 13, as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13 under Subpart 13.5 test program. This procurement is 100% set aside for small business. This CBD announcement constitutes the only solicitation issued. A separate written solicitation will not be issued. Request for Quotation (RFQ) 618-146-01 entitled "Workstations for Radiology" is issued for Brand Name or Equal supplies. It has been determined that the Government's minimum needs can best be satisfied by Brand Name or Equal supplies. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27 and VAAR 97-8. NAICS Code 339112 and the small business size of 500 employees applies. Offeror's proposals are hereby being requested in response to requirements under a firm fixed price contract and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit proposals on behalf of the contractor. VA Medical Center, Minneapolis, MN has a requirement for brand name or equal Vital Images, Inc. Plymouth, MN Workstation (and related components) for Department of Radiology/Imaging. The brand name components, to which an Equal may be compared, are as follows: Quantity one (1) VITREA2PNT, Vitrea 2 (for NT) Software, including: User, Training & System Administration Manuals and Computer: Dual Pentium III 850 MHz Processor, three (3) 18 GB SCSI 10K RPM with RAID 5 configuration, 1 GB RAM, 3D Graphics Board, Flat Panel Monitor, Modem, Internal CD-R, Mouse, Mousepad & Keyboard, one (1) year System Maintenance, two (2) two-day applications training sessions, and installation to full operation. Functional capabilities: must allow 2D and 3D visualization and analysis of CT and MRI data; dedicated workstation must have ability for virtual endoscopy and bronchoscopy and a variety of vascular reconstructions and the potential for cardiac scoring. Price proposal will include all costs involved for the system including shipping charges. All items provided will be Y2K compliant. All responsible sources who can meet the VAMC's requirements and provide items as listed above may respond to this solicitation by submitting price and technical proposals on their company letterhead or bid form. Delivery will be made to the VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417. Vendor to state delivery date period after notification of award. Proposals should address number of days for delivery after receipt of order; offerors to respond to evaluation factors of Technical Capability and Past Performance (which are equal in weight to each other) and Price. Technical Capability and Past Performance, when combined, are approximately equal to Price. Past Performance will be responded to by providing information on the past three (3) most recent installations, with name and telephone number for references on those projects. The Government intends to make award to the offeror that is most advantageous to the Government's criteria. Evaluation criteria will include, but not be limited to: evaluation of descriptive literature, ability to meet the Government's requirement, delivery time frame, warranty, past performance history, and price/and price related factors. The Government may award a contract on the basis of initial offers received, without discussions. Therefore, each initial offer should contain the offeror's best terms from a cost/price and technical standpoint. The Government will award a single aggregate contract contingent upon best value and the offeror's ability to meet all requirements. VA staff members are solely responsible in determining acceptability of equal products. Offers are due June 5, 2001, at 2:30 pm. local time.
