Description

This Request for Quotation (RFQ) is prepared in accordance with = Federal Acquisition Regulation (FAR), Part 12, Commercial Services and = FAR Part 13, Simplified Acquisition Procedures. The purchase order = resulting from this RFQ will be a firm-fixed priced purchase order. The = place of performance for services under this RFQ is the Internal = Revenue Service, Fresno Service Center 5045 E. Butler Avenue Fresno, CA = 93888. The contractor shall provide all labor, personnel, materials, = supplies, tools, equipment, transportation and incidentals necessary to = accomplish the removal and disposal of existing carpet tiles, = replacement and installation of new carpet tiles, and clean-up of the = five foyers of the Internal Revenue Service (IRS) Fresno Service Center = (FSC). The location and dimensions for each of the foyers is as = follows: (1) Butler Avenue Main Entrance -- 28.5sq ft x 7.5sq ft (214 = square feet). (2) South East Entrance -- 45.5sq ft x 7.5sq ft (342 = square feet). (3) South West Entrance -- 45.5sq ft x 7.5sq ft (342 = square feet). (4) North East Entrance -- 45.5sq ft x 7.5sq ft (342 = square feet). (5) North West Entrance -- 45.5sq ft x 7.5sq ft (342 = square feet). Total approximate square footage of the five foyers is = 1,582. The contractor shall provide 12x12, 12x30, or 24x24 commercial = grade, slip resistant, exposed cord rubber carpet tiles. The carpet = tiles shall be the same as or equal to Marley Flexco Flex-tuft rubber = tile. The required color is a dark tweed (dark grays, light grays with = black, similar to the Marley Flexco color Tweed). The Contractor shall = provide tile samples with their offers for inspection by the = Contracting Officer and COTR for inspection prior to award of this = purchase order. The purchase order resulting from this RFQ will have a = period of performance of approximately 15 working days from the date of = award. Requests for RFQ packages shall be submitted on company = letterhead and mailed to the address below or faxed to 415-575-7440. = Telephone requests will not be accepted.
