- This is a combined synposis/solicitation in accordance with the format in FAR 12.6, and FAR 13.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The Request For Quotation (RFQ) number is 539-01-17. The performance period is from date of award though September 30, 2001, with three one-year options to renew. A firm fixed price contract will be awarded. The NAICS Code is 811212. The size standard is $18 million in annual sales. This solicitation incorporates the provisions and clauses in effect through Federal Acquisition Circular 97-26. This procurement is unrestricted but is potentially reserved for HUBZone 8a, 8a, HUBZone Small Business, Very Small Business and Small Business participation. The contract will be awarded in the cascading order above in order to provide maximum practical opportunities for small and small disadvantaged businesses. All responses from responsible sources will be fully considered. In accordance with Federal Acquisition (FAR Subpart 19) any award resulting from this solicitation will be made on a competitive basis to an eligible HUBZone 8a concerns, provided that a minimum of 2 competitive offers are received form eligible HUBZone 8a concerns. If a minimum of 2 offers from eligible HUBZone 8a concerns are not received, the award will be made to eligible 8a concerns, and so on through the cascading order above. If a minimum of 2 competitive offers from qualified small businesses is not received, the RFQ will be competed on a full and open basis among responsible firms. This RFQ is being issued to provide full maintenance and repair service on the following Canon color copier equipment at the VA Medical Center, 3200 Vine Street, Cincinnati, OH 45220. The following equipment is located in the Medical Media Department: Canon Model 2400, serial #NKZ00327 color copier, Colorpass 800 module, Model F-1 Editor, Model BD B-2 Interface Board, Model A1 EP-ED Board, and Model RDF-E2 document feeder. The contract shall cover all maintenance and repair including parts (including drums), labor and travel. Response time for service calls shall be within 48 hours. The maintenance charge shall include copy charges of at least 10,000 copies per month. The current monthly copier volume is approximately 17,600 copies. Quotes must state the cost per component, per month. It must also state how many copies are covered under the agreement and what the excess per copy charge would be. Quotes must include a completed copy of the provision 52.212-3 Offeror Representations and Certifications-Commercial Items (APR 2001), the DUNS number in accordance with 52.204-6 Contractor Identification Number-Data Universal Numbering System (JUN 1999), and VAAR 852.219-70, Veteran-Owned Small Business (DEC 1990).
