Description
The Office on Women's Health (OWH) through the Program Support Center, Division Acquisition Management intends to award a twelve (12) months Simplified Acquisition' for Professional Services. The contractor shall be responsible for the developing and implementing a pilot comprehensive systems of HIV/AIDS health care services for incarcerated and newly released minority women living with HIV/AIDS/STDS. The results will be accomplished in two phases as follows: Phase 1 1 -- Developing a pre-release process of administering need assessment to document health care service linkage and access needs of incarcerated minority women living with HIV/AIDS/STDS due to be released. 2 -- Developing a discharge planning process for incarcerated minority women living with HIV/AIDS/STDS. 3 -- Developing a transition planning (using MOU) process linking newly released minority women living with HIV/AIDS/STDS with providers of health care services. 4 -- Developing a peer educator training to train a formerly incarcerated minority women living with HIV/AIDS/STDS to serve as core staff for this project. 5 -- Formalizing a service linkage and access process (using MOA) through collaborative partnerships with providers of health care services, such as HIV/AIDS primary medical care, high risk medical services, emergency/acute care, OB/GYN and family planning, dental and optical services, mental health services, psychotherapy, medical screenings (hemoglobin, mammograms, etc.), clinical trials, substance abuse treatment, nutrition and fitness services, and spirituality. PHASE 2: 1. Providing pre-release, discharge planning, transition planning via linkage and service access to health care services for 15 incarcerated and/or newly released minority women living with HIV/AIDS/STDs. 2. Attending AND making workshop presentation or poster presentation on project highlights with distinctions on women-centered service delivery needs for incarcerated and newly released minority women.
