Description
This is not a solicitation announcement. This is a Sources Sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (HubZone 8(a), 8(a), HubZone, Small, Small disadvantaged and Large Business relative to NAICS Code 811219, size standard of $5.0 million average annual receipts for preceding three fiscal years. Responses to this sources sought synopsis will be used by the government to make appropriate acquisition decisions. After review of the responses to this announcement a solicitation announcement will be published in the CBD. Responses to the announcement are not adequate responses. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sole source announcement. The New Mexico VA Health Care System (NMVAHCS), Albuquerque, NM, VISN 18, is seeking sources for a potential contract to provide all necessary labor, materials, parts and transportation required to maintain and repair Computer Tomography, Model Somatom Plus with workstation (magicview), Nuclear Gamma Camera, Model ZLC 750S, Nuclear Gamma Camera Computer, Scintiview computer (3256), Nuclear Camera ECG, MFG. AMRC Model Accusyne II, Fluoroscopier/Radiographic, Mfg. Siemens Model Sireskop SX Fluro (31116680), Cardiology Cath Laboratory, Mfg. Siemens, Model 7166689x1373, Cardiology Cath Laboratory, Mfg. Siemens, Model Coroskop T.O.P. (1P1614562175), Cardiology Computer workstation, Mfg. Siemens Model Acom-M, Cardiology workstation, Mfg. Siemens Model Acom-PC. Required services will consist of scheduled PM inspections, the equipment shall be maintained in optimum operating condition and meet manufactures specifications for operation and performance. Contractor shall have access to and readily available manufacture replacement parts. Contractor shall keep the equipment covered by the contract in optimum operating conditions. The intended contract shall provide the Government the option to extend the contract in one-year increments for a base year and four option years. Responses to this notice should include company name, address, and point of contact, cage code, size of business pursuant to North American Industrial Classification System (NAICS). This is not a request for proposal and in no way obligates the Government to award a contract. Responses to this notice shall be mailed to the attention of Kathryn Ogilvie (90C), New Mexico VA Health Care System, 1501 San Pedro Dr. SE, Albuquerque, NM 87108. Telephone and/or e-mail responses will not be accepted. Responses must be received in writing no later than 3:00 PM on June 12th and may be faxed to (505) 256-2894. Firms responding to the announcement, which fail to provide all of the required information cannot be considered by the government in making the acquisition decision, which is the intent of this synopsis. No solicitation is currently available; all firms responding to this notice are advised that their response to this notice is not a request to be considered for a contract award. If a solicitation is issued it will be announced at a later date in the CBD, and all interested parties must respond to that solicitation
