Description
-This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and Part 13, Simplified Acquisition Procedures, as supplemented with additional information contained in this notice. This notice incorporates provisions and clauses in effect through FAC 97-25. NAICS Code 561320 applies and the small business size standard is $10 million. This announcement constitutes the only solicitation. A written solicitation WILL NOT be issued. This solicitation is issued as a Request for Quotations (RFQ). RFQ 544-076-01 applies. WJBD VA Medical Center has determined an urgent requirement for two (2) Temporary Diagnostic Radiological Technologist to work varying shifts Sunday through Saturday 8 a.m. 4:30 p.m, 4 p.m. 12 midnight, and 12 midnight 8 a.m. (40 hours per week) based on the needs of the department for 13 weeks of full-time work. Service start date is not later than July 2, 2001. Each technologist must be certified under American Registry of Radiologist Technologists (AART) and have a minimum of two (2) years experience in a hospital setting. Contractor's proposal is requested and shall contain the name, title and signature of person authorized to submit proposals/bids on behalf of the contractor. Cost proposal shall be broken down in such a way that each shift and the unit price per hour as well as totals are shown. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form. The following is a list of (A) Duties and responsibilities: (l) Corroborates patient's clinical history with procedures ordered.(2) Prepares patient for procedures; provides instructions to obtain desired results, gain cooperation and minimize anxiety.(3) Provides patient education regarding exam.(4) Respects confidentiality and follows established policy regarding patient records.(5) Assumes responsibility for specific job requirements and reports any equipment malfunctions.(6) Immobilizes patients as required for appropriate examination.(7) Assumes responsibility for providing for the physical and psychological needs of patients during procedures. (8)Initiates basic life support action when necessary.(9) Understands and applies patient relations skills.(10) Assists in the day to day operation of the department in routine housekeeping and equipment care that the department is responsible for.(11) Selects and operates radiographic equipment such as the c-arm and portable units; image intensified fluoroscopy, and/or assorted accessories to successfully complete the procedures. Experience with DR & Kodak CR a plus but not required. (l2) Applies principles of radiation protection to minimize exposure to the patient, self and others. (l3) Evaluates radiographs or images for technical quality, ensuring proper identification is recorded. (l4) Understands various methods and is capable in performing venipuncture. (l5) Prepares, identifies and administers contrast media and/or medications as prescribed by a physician. (l6) Assists with fluoroscopic and various interventional procedures. (17) May perform non-interpretative fluoroscopic procedures, as appropriate. (B) Knowledge required: (1) Imaging equipment; mobile equipment recording media and techniques such as video recorders, video tubes, etc.; radiographic equipment-various manufacturer types and operation of each. (2) Radiation protection; personnel monitoring and occupational exposures; patient protection. Contractor's employees must be present at the VA and must be performing the required services or the contract cost will be decreased accordingly. The 30-minute lunch period will not be compensated. Monitoring of contractor's time shall be demonstrated through sign-in/sign-out sheets upon reporting to and departing from work. Payments to contract personnel for any leave, including sick leave, holiday, or vacation time are the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor's policies. The contractor shall be responsible for protecting the personnel furnishing services under this contract. The contractor shall provide workers compensation, income tax withholding, social security payments, etc. The government may evaluate the quality of professional and administrative services provided, but retains no control over the professional aspects of services rendered. Substitutions shall have appropriate qualifications and VA reserves the right to request replacement, if necessary. Award preference will be as follows: 1) award to the lowest priced, responsible, responsive offeror who offers at least one technician start date of June 18, 2001 or earlier; 2) if no qualified contractor offers start date of stated in #1, then award to the lowest priced, responsible, responsive offeror who offers a start date of June 25, 2001 for both technicians; 3) if no qualified contractor offers award dates specified in #1 or #2, then to the lowest priced, responsible, responsive offeror who meets the start date of July 2, 2001 for both technicians. Award may be made with or without discussions. All responsible sources that can meet the requirement and provide services as listed above may respond to this solicitation by a submitting cost proposal on their company letterhead or bid form.
