- 17. Louis Stokes Cleveland VAMC is seeking sources that can provide on-call surgical laser services. On-call laser services must be available at a minimum twenty four (24) hours notice. Types of lasers required include: CO2 Laser, Nd:YAG Surgical Laser, KTP YAG Laser, and Ho:YAG Surgical Laser. Laser services provider shall include: providing qualified technician(s); delivery of laser to hospital one (1) hour prior to scheduled procedure; assisting biomedical technician in examining equipment prior to use; prepare laser for use in the operating room; set up laser for use by surgeon; assist surgeon on laser procedure settings; provide consumable items (e.g. fibers); serve as laser safety officer; maintain records of the procedure as required by JCAHO; provide copies of the records of the laser procedure to the Nurse Manager; provide all safety equipment, glasses and signage; remove the equipment from the operating room after use. Surgical Laser procedures shall be performed in accordance with all applicable requirements and guidelines, which shall include, the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO), and when applicable, the Louis Stokes Cleveland Dept. of Veterans Affairs Medical Center. The contractor shall include annual familiarization training of surgical staff on laser procedures. Louis Stokes Cleveland VAMC performs an estimated 15 30 laser procedures per year. Interested in trade-in of three (3) pieces of the following equipment: 1) Heraeus Lasersonics Inc, YAG Surgical Laser, model 8900; 2) Laser Scope, Laser/Coagulation Scope Apparatus, model KTP532; and 3) Sharplan, CO2 Surgical Laser, model Sharplan 1060. This is a request for information only! This is not a request for proposal (RFP). The VA does not intend to pay for information received. Requests for copies of a solicitation shall not be honored or acknowledged. Information should be forwarded to the Contracting Officer by June 8, 2001. VA will review information from which a requirement may or may not result.
