- The Social Security Administration (SSA) is seeking offers for = solid waste collection, disposal and recycling services at the Social = Security Administration, Wilkes-Barre Data Operations Center, 1150 East = Mountain Drive, Wilkes-Barre, PA. This is a combined = synopsis/solicitation for commercial items (as defined by FAR 2.101) = prepared in accordance with the format in Federal Acquisition = Regulation (FAR) Subpart 12.6, as supplemented with additional = information included in this notice. This announcement constitutes the = only solicitation. A written request for quotations will not be = issued. This acquisition is subject to FAR Subpart 13.5 regarding the = application of simplified acquisition procedures. This acquisition is = issued as Request for Quotation SSA-RFQ-01-0258. All responsible = business sources may submit an offer that will be considered. The = NAICS code is 562111. The small business size standard is $10 million. = This is NOT a small business set-aside. The solicitation document and = incorporated provisions and clauses are those in effect through Federal = Acquisition Circular 97-25. Offers in response to this RFQ shall be = submitted by 3:00 p.m. local prevailing time, Wednesday, June 20, 2001. = I. OBJECTIVE: The objective of the contract is to obtain recycling and = trash and debris removal and disposal services. II. SCHEDULE: Monthly = Rates -- Firm-fixed prices which include all services listed below in the = scope. A. Monthly price for solid waste collection. B. Monthly = Percent of Reimbursement for Recyclable Materials or cost to the = Government to remove/recycle per dumpster. Repeat this pricing scheme = for all 5 years. III. SCOPE: A. The contractor shall furnish all = labor, materials, supplies and equipment necessary to accomplish the = following: 1) Trash Removal Service: Remove and dispose of all trash = and garbage, which includes a variety of unsalvageable waste materials; = 2) Debris Removal Service: Remove and dispose of all debris and wood = which includes, but is not limited to: plaster, wallboard, brick, = stone, ceramic tile, scrap lumber, crates, boxes, skids, etc., which = may contain nails, pieces of metal and other non-burnable material. = Grass cuttings, tree trimmings, leaves, pine straw, limbs, stumps, and = street sweepings are also included; and 3) Removal of Recyclable = Materials: Remove all accumulated recyclable materials which includes: = mixed paper, corrugated cardboard, glass, plastic, and aluminum. = Materials intended to be recycled cannot be landfilled or disposed of = in any other manner. Paper material shall not be sold for resale as = files or records or for the information it contains. The contractor = shall pay the Government for the value of the recyclable materials. 4) = Excluded is collection and disposal of hazardous wastes and = infectious/medical wastes. B. History of Collections: 3-year average: = Aluminum 4.09 ton (but 1999 was only .26 ton); mixed paper 6.35 ton; = corrugated cardboard 24.82 ton; glass .75 ton; plastic .25 ton; and = waste garbage 399.58 ton. IV. EQUIPMENT: The contractor shall provide = the following equipment: 1) 8 cubic yard capacity self-contained = compaction container located at the cafeteria dock on the east side of = the building; 2) 25 cubic yard capacity recycling roll-off container = located on the west side of the building; and 3) 40 cubic yard capacity = compaction container with cart dumping container, compaction unit, and = control unit located on the west side of the building. All containers = must be aesthetically pleasing and painted to eliminate any unsightly = graffiti, rust, etc. All equipment must be maintained in good = operating condition. Equipment must be repaired or replaced within 24 = hours of problem notification. The Project Officer will give direction = as to the locations where containers may be placed. V. FREQUENCY OF = SERVICE: Daily: Pickups for the cafeteria compaction container (8 cy) = shall be performed daily, Tuesday through Saturday before 7 a.m. = Weekly: Pickups for the recycling container (25 cy) shall be performed = once a week on Wednesdays before 7 a.m. Weekly: Pickups for = trash/garbage (40 cy) compaction container shall be performed as needed = and as requested by the Government, not to exceed one call in a 7-day = period. Pickups shall be completed within 24 hours of notification. The = project officer or his/her designee will approve delays if caused by = severe inclement weather. When a scheduled collection day falls on a = holiday, the collection(s) shall be made on the next Government = workday. VI. IDENTIFICATION OF VEHICLES: All vehicles shall bear the = name of the company. VII. ABNORMAL QUANTITIES OF WASTE: Excess waste = beyond the normal quantity resulting from holidays or other reasons = shall be collected and disposed of by the contractor at no additional = cost. VIII. TRASH & DEBRIS LITTERED DURING REMOVAL: The contractor = shall clean up trash and debris scattered as a result of operations on = grounds, lawns and roadways in the vicinity of the pickup point. = Contractors shall not separate and/or segregate trash or debris on = Government premises. IX. INACCESSIBLE CONTAINERS: The contractor shall = make all pickups as scheduled. The contractor shall notify the Project = Officer or his/her designee if a pickup location is obstructed. X. = ADHERENCE TO LAWS AND REGULATIONS: The contractor shall comply with all = applicable local, state, and Federal laws and regulations. The = contractor shall pay all fees associated with disposal. Copies of any = licenses/permits shall be made available to the Government upon = request. XI. PERIOD OF PERFORMANCE: The contract will include a base = period of twelve months and four (4) 1-year options, if exercised. The = term of the contract shall be for 60 months, as follows: Base Year from = July 1, 2001 -- June 30, 2002; Option Year I from July 1, 2002 -- June = 30, 2003; Option Year II from July 1, 2003 to June 30, 2004; Option = Year III from July 1, 2004 to June 30, 2005; and Option Year IV from = July 1, 2005 to June 30, 2006. XII. PLACE OF PERFORMANCE: SSA, = Wilkes-Barre Data Operations Center, 1150 East Mountain Drive, = Wilkes-Barre, PA. 18702-7978. XIII. GOVERNMENT-FURNISHED SERVICES: The = Government will furnish utility services at existing outlets. All = recyclable materials will be pre-sorted (to the maximum extent = possible) by the Government and placed in the pick-up containers in = separate bags. XIV. CONTRACT ADMINISTRATION: A. Designation of = Contract Specialist: Regina Dulaney, Contract Specialist, Division of = Construction and Facilities Contracts, will administer the contractual = aspects of this contract. Changes in the scope, contract cost, price, = quantity, quality or delivery schedule shall be made only by the = Contracting Officer by a properly executed modification. All = correspondence which concerns the terms or conditions of this contract = shall be submitted directly to the Contracting Officer at the following = address: SSA, Office of Acquisition and Grants, 1710 Gwynn Oak Avenue, = Baltimore, MD 21207, Telephone Number -- 410-965-9508. B. Designation = of Project Officer: Vince Holland , 570-831-1316 is the Project = Officer. Debbie Heil 570-831-1329 is the Alternate Project Officer. The = project officer or alternate shall be responsible for the technical = aspects of the contract. The project officer/alternate cannot make any = changes that affect the contract amount, terms or conditions. The = contracting officer is the only authorized agent of the Government = under this contract. Only the contracting officer has authority to: (1) = direct or negotiate any changes in the Statement of Work; (2) modify or = extend the period of performance; (3) change the delivery schedule; or = (4) otherwise change any terms and conditions of this contract. XV. = REPORTING REQUIREMENTS: A. Receipts Identifying Recyclable Materials: = Receipts shall identify all materials by type and quantity (and grade, = if appropriate). The Project Officer may request records of what was = transported from the facility and receipts obtained. The Project = Officer or designee shall approve of the methods of identifying = materials prior to receipts and of keeping records of these = transactions prior to starting work; B. Annual Reporting: 1. The = Contractor shall be responsible for tracking and completing the annual = report required by Plains Township for all trash and recycled materials = generated at and removed from the facility. The Contractor shall = provide a copy of this report to the Project Officer by February 15, = for the prior calendar year. 2. The Contractor shall provide to the = Project Officer a summary report of the monies paid to or by the = Contractor for the recyclable materials in the prior year. The summary = shall be broken down into the amount-received from/paid for the = disposal of each type of material: The report shall be submitted by May = 31of each year for the prior contract period. 3. Pickup Tickets: At = Contractor shall provide pickup tickets at the time of each removal. = The pickup tickets shall include the contractor's name, date and time = of pickup, type of removal (trash, recycling or debris) and the = signature of the contractor's employee. One copy for the Project = Officer shall be left at the control center in the main lobby of the = facility. The contractor shall retain one copy for its own records. = XVI. CONTRACTOR PERFORMANCE REPORTS: Contractor Performance Information = will be maintained in accordance with OFPP Policy Letter 92-5 Past = Performance Information and FAR 42.15. XVII. TERMS AND CONDITIONS: The = provisions of FAR 52.212-4 Contract Terms and Conditions -- Commercial = Items (May 1999) (tailored as follows) and FAR 52.212-5 Contract Terms = and Conditions Required to Implement Statues or Executive = Orders -- Commercial Items (Feb 2001) are incorporated by reference. FAR = 52.212-4 (a) Inspection/Acceptance is tailored as follows: A. QUALITY = ASSURANCE: The Government will perform random inspections on the = services provided. Inspections will include all aspects of services = required by the contract. A 100% Acceptable Quality Level will apply. = All unacceptable services will be rejected and deficiencies corrected = by the Contractor. FAR 52.212-5 applies as follows: (a)(1) and (2); = (b)(1), (5), (11), (12), (13), (14), (15), (20), (24); (c)(1), (2), = (3); (d); (e). B. OPTION TO EXTEND SERVICES: FAR 52.217-8 (Nov 1999); = C. OPTION TO EXTEND THE TERM OF THE CONTRACT: FAR 52.217-9 (Mar 2000) = [fill-ins: #1-at any time during the period of performance; #2-60 = days]; D. INSURANCE: FAR 52.228-5 (Jan 1997) (the contractor shall = maintain those amounts of insurance which are prudent for the = industry); E. WAGE DETERMINATION: WD #94-2453 Rev. 13 dated 9/18/2000 = shall apply to services performed in Luzerne County PA. The labor = category of heavy truck truckdriver (31363) as defined in the = Department of Labor Directory of Occupations, 4th Edition, applies. The = contractor must submit a listing of the number and types of employees = utilized under the contract for the purpose of seeking annual price = adjustments. XVIII. SITE VISIT: We will conduct a site visit on June = 11, 2001 beginning at 9:30 am. Interested parties should contact Regina = Dulaney on 410-965-9508 or by email at gina.dulaney@ssa.gov. Offerors = are encouraged to visit the site and take steps to determine the nature = and location of the work, and the general and local conditions that may = affect the work or the cost. Failure to do so will not relieve offerors = from responsibility for properly estimating the difficulty or cost of = performance. XIX. INSTRUCTIONS FOR SUBMITTING QUOTES: The provisions at = FAR 52.212-1 Instructions to Offerors -- Commercial Items (Oct 2000) are = incorporated by reference. Offerors shall follow these instructions. = Special attention is directed to the following paragraphs: (b)(4) does = not apply; (b)(6) Pricing will be on a fixed monthly rate and fixed = percentage for recyclable materials for each period of performance; = (b)(10) All information required by this paragraph shall be submitted. = Experience should be limited to current projects and/or projects = completed within the past 3 years; (c) Period of acceptance for offers = is 90 calendar days; (d), (e), (h) and (i) do not apply. Completed = Representations and Certifications as required by FAR 52.212-3 Offeror = Representations and Certifications -- Commercial Items (Apr 2001) shall = be submitted with the offer. Offers which fail to include all required = information may be rejected without further consideration. XX. = EVALUATION CRITERIA: Award will be made on a best value basis taking = into account the following factors: price, experience, past = performance, and proposed percentage for recyclable materials. The = Government requires the monthly price to be broken down by service to = the 8 cy compaction container, 25 cy recycling container and the 40 cy = compaction container for evaluation purposes. The Government may award = to other than the lowest priced offer based on a trade-off among these = factors. XXI. AWARD: A written contract or notice of award will be = mailed or otherwise furnished to the successful offeror, within the = timeframe for acceptance of offers, shall result in a binding contract = without further action by either party.=20
