Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the Format in Federal Acquisition Regulation (FAR), Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and is being issued as a Request For Quotation (RFQ) and a written solicitation will not be issued. The solicitation number is 549-1-RFQ-144. The solicitation document, incorporated clauses and provisions are those in effect through Federal Acquisition Circular 97-26 and Veterans Affairs Acquisition Regulation (VAAR), VA Acquisition Circular 97-7. The Contracting Activity located at the VA North Texas HealthCare System (VANTHCS), 4500 South Lancaster Rd., Dallas, TX 75216, requires an IMAGING TABLE SYSTEM. Minimum specific requirements are as follows: Radiolucent imaging surface that is body-length, with weight capacity of 450 lbs., 4-way table movement (longitudinal and lateral), Lateral tilt and head tilt (Trendelenberg) capability while retaining 4-way table movement), no table support structure beneath the entire length of the table (this would block access to X-ray tube), capability to attach supports for fixed retractors, auxiliary clamps, radiolucent single arm board 21" wide. Price to include: installation, training and shipping charges (FOB Destination). This solicitation incorporates by reference FAR Clauses and Provisions, FAR 52.212-1, 52.212-3, 52.212-4, and addenda (52/214-21, 52.227-19. 52.233-2, 52.233-3, 852.270-4). These clauses and provisions are available on the Interest Web at the following URL: http: //www.arnet.gov/far. For efficiency and cost effectiveness, it is the VA's intent to award a single PO to the one responsive and responsible, whose offer is most advantageous to the government price and other factors considered. For interested parties who are able to provide the requested item (s), are encouraged to submit sealed quotation on Company Letterhead, along with completed FAR Provision 52.212-3, "Offeror Representations and Certifications", and descriptive literature, Offers must be received on or before 1:00 p.m., Dallas, TX, local time, on 6/21/01. Offers must reference the solicitation number and sent to the attention of the Contracting Officer.
