Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation, number N00259-01-T-0033, is issued as a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-26. The Naval Medical Center San Diego intends to award a sole source, firm-fixed priced purchase order to Visx, Inc., 3400 Central Expressway, Santa Clara, CA 95051 for 200 EA Ophthalmic Laser Vision Key Cards, PN: 0030-3057-07, to be used with the Star S3 US Lasik PRK Excimer Laser, Serial# 3131. Items to be delivered by 29 Jun 01 to the Naval Medical Center, 34800 Bob Wilson Drive, Receiving Division, Loading Dock One, San Diego CA 92134. This acquisition incorporates the following FAR clauses: 52.212-1 Instructions to Offerors -- Commercial Items (Oct 95); 52.212-3 Offeror Representations and Certifications -- Commercial Items (Oct 95) must be completed and submitted along with the proposal (a copy can be obtained by contacting Glenda C.
