Description

Provide Transitional Residency with supporting services for the HCHV (Health Care for Homeless Veterans)program for the VA Medical Center, Bay Pines, Florida. Contractors must have facilities available in the areas of Bradenton, Florida; Ft. Myers, Florida; Palmetto, Florida; and Sarasota, Florida. Firms will be evaluated on the basis of experience, education/training, past performance, resources, location, price and other factors. Solicitation will be available on or about 6/18/01 and must be requested in writing. Faxed requests will be accepted at (727) 398-9536. Contract will be for a base year period from the date of award to 9/30/01, with the Government having the option to extend the contract for four (4) one-year periods. The contract will be awarded as a fixed-price Commercial Item acquisition in accordance with FAR Part 12. This is a 100% total small business set-aside. NAICS 623220 applies.
