Description
The 10th Air Base Wing, Operational Contracting Office (LGCA), = USAF Academy, CO, 80840-2315 is issuing this synopsis for solicitation = F05611-01- T-0395. This is a combined synopsis/solicitation for = commercial items prepared in accordance with the format in Subpart = 12.6, as supplemented with additional information included in this = notice. This announcement constitutes the only solicitation; proposals = are being requested and a written solicitation will not be issued. This = solicitation is issued as a Request for Quotation. This solicitation = document incorporates provisions and clauses applicable to this = solicitation that are in effect through Federal Acquisition Circular = 97-24. STATEMENT OF WORK RADON INVESTIGATION FOR MILITARY FAMILY = HOUSING, USAF ACADEMY COLORADO 16 APRIL 2001 The contractor shall = provide all labor, material, equipment, tools, and transportation = necessary to deploy, conduct, and collect long-term radon investigation = at 800 designated Military Family Housing (MFH) units at the USAF = Academy, Colorado. The work will include submission of a work plan, = installation of radon test kits in buildings, collecting test kits and = reporting on test kit results. Installation of radon test kits: 800 = kits are required for installation in the following units: All MFH = units in Pine Valley and approximately 100 units in Douglas Valley. = Bioenvironmental Engineering and Housing Maintenance will choose 100 = units to be tested in Douglas Valley according to the past radon = measurements. The housing residents will be informed about this radon = re-assessment by base newspaper and/or direct mail prior the placement = of test kits. Bioenvironmental Engineering will do this prior = notification with some assistance from Housing Maintenance. = Installation of test kits shall be completed within 14 calendar days = after receipt of purchase order. Collecting of test kits: The kits = shall be collected after 90 calendar days, and sent to a certified = laboratory for analysis. Reporting on test kit results: The contractor = is required to prepare a technical report summarizing all testing = actions employed and the testing results. Submit a hard and electronic = (Microsoft Access database) copy of the report that includes the = following parameters: Location (MFH unit address), Unit Style (Single, = Duplex, or 4-plex), Basement Style (Enclosed, Walk-out, or = Garden-level), Radon Mitigation Unit Present (Yes or No), Sample = Placement Date, Sample Collection Date, Result, and Uncertainty of = Result. (NOTE: Any additional recommended work under this contract, not = within the scope of this requirement, shall be solicited and awarded as = a new requirement.) The accuracy of the test shall have an uncertainty = of less than .5 Pci/L.
