Description
This is a combined synopsis/solicitation, for commercial items, prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NHLBI-PS-2001-012, and is issued as a Request For Quotation (RFQ). The solicitation/contract includes all applicable provisions and clauses in effect through the Federal Acquisition Circular 97-26. This requirement is a small business set aside. The North American Industry Classification System (NAICS) number is 334516, Size Standard 500 employees is designated for this requirement. The National Heart, Lung, and Blood Institute (NHLBI), intends to purchase an 8 Channel Preamp Decoupled Phased Array System, Model name: The NMSC-010E-1.5T, manufactured by Nova Medical, Inc., or equivalent. The NMS-010E.1.5T device is an array of receive-only. It is made of surface coils with preamplifiers designed for cardiac MR application. The array is designed to permit a flexible array geometry. The sizes and positions of individual elements may be varied. The surface coil elements are "de-tuned" to minimize inductive coupling between array elements. The effective noise figure of the de-tuned surface coil element and preamplifier is sufficiently low that the system is sample noise limited for typical body loading. The NMSC-010-1.5T device consists of the following units, (1) surface coils with cables, (2) pre-amplifier modules, and (3) preamplifier-to-scanner cable assembly. The cable lengths are designed to facilitate placement of the array elements on the chest, side, and back in the vicinity of the heart.The Array must be compatible with our GE Scanner. The NMSC-010-1.5T device consists of the following units, (1) surface coils with cables, (2) pre-amplifier modules, and (3) preamplifier-to-scanner cable assembly. The cable lengths are designed to facilitate placement of the array elements on the chest, side, and back in the vicinity of the heart. The commodities are as follows: Item#1 quantity two (2) each, catalog# NM1A15TGE, Ulra-Low Impedance Four Channel Preamp Module For Ge MRE Scanner; Item#2 quantity one (1) each, Catalog#NMAGE0048, 8 Channel Ge Phased Array Connector with supply Voltages/Cable Assembly; Item#3 quantity four (4) each, catalog#NMSC010EA15T, 1.5T Receive-Only Surface Coil Array Elements; Item#4 quantity four (4) each, 15T Receive-Only surface Coil Array Elements; Item#5 quantity eight (8) each, catalog#NMSC010EC15T, 15T Receive-Only Surface Coil Array Elements. The Government desired delivery schedule is (30) thirty days after receipt of order. The delivery point is F.O.B. Delivery, Bethesda, Maryland. This acquisition is conducted under simplified acquisition procedures. The provision for FAR Clause 52.212-1, Instructions to Offerors -- Commercial Items, applies to this acquisition. Evaluation procedures = are based upon the product to be purchased and installed. The provisions of FAR Clause 52.212-2, Evaluation -- Commercial Items is applicable to this acquisition. The award will be made to the lowest priced, technically acceptable quote meeting the Government's specifications. The contractor must submit descriptive literature of the product quoted and the itemized price list. The Government intends to evaluate offers and award a contract without discussions with contractors. Therefore, the initial offer should contain the offeror's best terms from a technical and price standpoint in order to meet all government specifications. However, the Government reserves the right to conduct discussions if later, determined by the Contracting Officer to be necessary. The Government may reject any or all offers and waive informalities and minor irregularities in offers received. The award will be made based upon adherence to specifications / requirements, lowest price, and delivery time after receipt of order. The contractor must include the DUNS number, a completed copy of the provisions of FAR Clause 52-212-3, Offeror Representations and Certification Commercial Items with its offer. The Provisions of FAR Clause 52.212-4, contract terms and conditions commercial items, = applies to this acquisition. The addenda to the clause reads as follows: The Offeror must provide descriptive literature inclusive of specifications as part of the quotation. The offeror must include in their quotation, the unit price, the list price, shipping & handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders Commercial Items Deviation For Simplified Acquisitions, applies to this acquisition.
