Description

Contractor to provide all necessary labor, tools, materials, tools and supervision necessary to remove and replace existing surgery suite seamless flooring with grades of resilient rubber floor covering, for the Omaha Division, VA Nebraska Western Iowa Health Care System, 4101 Woolworth Avenue, Omaha, NE 68105, in strict accordance with drawings and specifications. Scope of work includes preparing submittals for materials to be used, preparing site for demolition and removal of existing flooring, scheduling use of approximately two operating rooms and adjoining scrub rooms at one time. These rooms shall be completed prior to proceeding to the subsequent areas. The contractor shall allow the VA up to 24 hours to completely clean and sanitize each area after it has been inspected and turned over for occupancy before another set of 2 OR rooms can be out of service. There is a total of seven (7) operating room floors plus scrub rooms to be replaced. Installation of flooring is to be in accordance with manufacturer's printed installation instructions. Sheet flooring will have heat-welded seams, and integral cove base installed at intersection of floor and vertical surfaces. Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered. Contractor shall permit access to VA personnel and patients through other construction areas, which serve as route of access to such affected areas and equipment. Contractor to coordinate alteration work in areas occupied by VA so that Medical Center operations will continue during the construction period. RFP No. 636-45-01 has been assigned to this solicitation and will be available approximately July 9, 2001, and will be due back by approximately August 8, 2001. NAICS Code No. 235520 Floor laying, finishing or refinishing construction contractors, with a size standard of $11.5 million has been assigned to this solicitation.
