Description

The Great Lakes Acquisition Center (GLAC) is requesting quotations to provide Primary Care Medical Services through the establishment of a Community Based Outpatient Clinic (CBOC) in Gurnee, Illinois to beneficiaries of the VA Medical Center North Chicago, Illinois. This procurement is for a period of one-year from date of award with two one-year options. All responsible sources may submit a quote. Requests for a hard copy solicitation must be in writing. A charge of $5.00 per hard copy is required. Checks are to be made payable to: DEPARTMENT OF VETERANS AFFAIRS. No telephone requests will be accepted.
