Description

Project 623-01-006, RELOCATE SILVER TEAM TO BUILDING 53, at the Department of Veterans Affairs, VA Medical Center, 1011 Honor Heights Drive, Muskogee, Oklahoma 74401. Request for Proposal (RFP) 623-23-01 is issued for the following requirement: Provide all labor, materials and equipment to perform construction of the shelled space to relocate Silver Team/Special Exam Unit to the third floor of Building 53. The project will provide clinical facilities consisting of exam rooms with bathrooms, waiting area, physician office, chemotherapy, clerk office and medication room. All exam rooms will need lavatories installed. Construction will consist of walls, floors, ceilings, HVAC, ductwork, electrical, plumbing, fire protection and architectural items necessary for a complete build-out of the shelled space. Testing and Balancing of the system is necessary. This project will improve approximately 4,709 square feet of usable space. Since this space is unfinished, demolition will not be required. The project shall be constructed according to VA drawings and specifications. Construction must ensure that Patient Care Services are not impeded. Additionally, continual access must be maintained for all handicapped patients, visitors, and employees. Work is to be completed within 150 days from the start of the project. The North American Industry Classification System (NAICS) code is 233320, and the size standard is 17 million . Magnitude of this project is between $250,000.00 and $500,000.00 and is a HUB-ZONE set aside. Issue date of the solicitation is July 6, 2001. The government shall not be held responsible for the quality of faxed transmittals. The solicitation number (RFP number) shall be listed on the outside of your response. Proposals are due July 26, 2001.
