Description
This is an emergency procurement using combined synopsis/solicitation for commercial services (Scarce Medical Specialist Resources or Locum Tenens), for one full time UROLOGIST, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation will not be issued. All responsible sources may submit an offer for consideration. Offers shall be considered only if clear and convincing evidence is provided documenting that the offeror can meet the requirements of the Government. Solicitation number is 502-46-01 and it documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 97-27 and VAAR # 21. It is unrestricted and all responsible sources may submit an offer for consideration. The North American Industry Classification System code for Locum Tenens is 561330 and 621111 for physician offices with $5.0 million annual sales for small business limitation. As a minimum, for each candidate under consideration, submit a curriculum vitae, a minimum of three references (names and phone numbers), previous employers and documentation sufficient to complete credentialing and privileging process -- education, board certification status, relevant training, experience, pending or settled challenges to license, and status of any claims. VA Application form 10-2850 must be completed in its entirety and is available upon request at 318-473-0010 ext 2765 or 2202. Credentialing and privileging prior to acceptance is essential. Requirement is One (1) Board Certified or Board Eligible Urologist for approximately 3 months, commencing immediately. Services are required for the period July 9, 2001, through September 30, 2001. Extension of services beyond September 30 may be requested for a limited time upon mutual agreement and availability of funds. (1) Estimated 480 hours @ $______________ $ ____________. (2) Call-backs, estimated 20 each @ $ _____________ $ ____________. NOTE: This is an all-inclusive rate. No additional consideration will be provided for housing, travel, transportation, etc. This procurement requires all phase of Genitourinary services at this medical center in Alexandria LA. The contract urologist will provide support to one VA staff urologist. Schedule includes outpatient clinic Monday, Tuesday, Wednesday and Friday and surgery on Thursday, 8:00 AM to 4:30 PM, excluding holidays. The ten holidays observed by the government include New year's Day, Martin Luther Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas and any other day proclaimed by the President of the United States to be a national holiday. One physician for the duration of the contract term is required for continuity of care and credentialing. If a substitute is required due to emergency circumstance, any proposed substitute(s) shall have qualifications that are equal to or higher than the qualifications of the person being replaced. The Contracting Officer shall evaluate request for substitution of personnel and promptly notify the contractor in writing whether the request is approved or disapproved. No overtime, sick or holiday coverage/pay is included. Payment will be made for actual hours worked. Callback services will be limited, primarily to the contract physician's post-operative patient care. The contract may be changed by written modification. VA intends to award a firm-fixed price, non-personal services contract for Health Care Resources in accordance with Public Law 104-262 and the authority of Title 38 U.S.C. 8153. Responsibilities: Urology Physician is responsible for all facets of adult Urological/GU care as established by industry and VHA standards. He/She shall be in good standing with the profession with knowledge and abilities current to equipment, medication and techniques. The Board Certified/Eligible Urologist shall provide comprehensive health services including inpatient, surgical, and outpatient clinic with all phases of GU treatment and care including routine and malignancy, and appropriate surgical procedures (cystoscopy, prostate biopsy, TURP, etc.). Primary responsibility is daily clinic with approximately 30-35 patients scheduled each day. Other duties include computer documentation of findings and impressions and accession of reports and records (all phases of automated information security apply), writing prescriptions restricted to VA formulary, and participation in the Tumor Board when a GU cancer patient is presented. This medical center is one of a network of VA Medical Centers (VISN 16) sharing medical expertise and patients may be referred to or received from another VA for specialized services. Contract physician shall communicate with other professionals to ensure quality patient care. Documentation and services shall be performed in accordance with all VAMC established principles, ethics, standards, rules and regulations as set forth in VHA Directives, Medical Staff Bylaws, Medical Center Memorandums, AMA, and JCAHO standards. The Director of Information Management or his designee will address computer usage and security requirements. All data is proprietary and shall not be shared or manipulated in any manner. Contract monitoring and verification of services provided shall be under the direction of and monitored by the Contracting Officer's Technical Representative, the Director of Specialty Care Healthcare Line (SC HCL) and the Chief of Staff. Any incidents of contractor non-compliance evidenced by the monitoring procedures shall be reported immediately by the COTR to the Contracting Officer. Teamwork and ability to work independently without supervision are essential. The Chief of Staff is the final authority for services provided under the terms of this solicitation and any resulting contract and shall maintain an ongoing utilization review. All rules, regulations and laws applicable to privacy of patient, documentation and records apply. VA reserves the right to refuse approval under this contract or to dismiss or terminate the contract at any time for unsatisfactory performance or noncompliance or failure to conform in appropriate and professional areas of skills, knowledge, actions, personality and interactions or for any other valid reason considered by the Contracting Officer to be objectionable. Preferred payment method is weekly in arrears using Government purchase card but other arrangements may be made. PAYMENT IS ON AN HOURLY BASIS FOR ACTUAL HOURS WORKED at this medical center in a work atmosphere. Logs of work-hours (certified sign-in/sign-out record maintained by Specialty Care HCL) will be faxed to Contractor weekly for calculation and billing. Invoice shall be faxed to 318-483-5063 for credit card information. Payment shall not exceed 40 hours per week. Callbacks shall be listed as a separate line item. The hourly rate is an all-inclusive rate and no additional consideration will be made. The Contractor shall accept payment for services rendered under this contract as payment in full and will not bill the veteran or a third party insurer for any services covered under this contract. The contract physician shall provide the same professional level of care to VA patients as he/she would provide to any other patient under his/her care. Qualifications: Contract Urologist shall be licensed in a state, territory, or commonwealth of the United States or District of Columbia with current full, unchallenged and unrestricted license; shall be board certified or board eligible with four or less years post residency. The contract urologist shall speak, understand, read and write the English language fluently; be COMPUTER LITERATE and comply with all aspects of computer security, preferably with specific knowledge of Computer Patient Record System (CPRS)/VISTA electronic medical records documentation access and input. Specific instruction of VA electronic records systems will be provided; however, basic computer skills are a must. He/she shall be a US citizen and possess required elements to be credentialed and privileged in accordance with VA regulations. The contract physician shall have current Basic Life Support Certification, current DEA certification, documentation of current TB skin testing and record of Hepatitis B vaccination with current titer or signed declination form. Physician credentials/ qualifications are subject to review and approval of the Medical Center Chief of Staff and Medical Center Director. The Alexandra VAMC Credentialing Officer requires all requested documentation necessary to credential and privilege the physician prior to acceptance at this facility. Individuals listed on the OIG List of Excluded Individuals, the List of Parties Excluded from Federal Procurement and Nonprocurement Programs and individuals required to submit VETS 100 Reports but failing to do so, are excluded from participating in this procurement. The contractor shall be responsible for protecting contracted physician assigned to this facility. The contractor shall provide worker's compensation, professional liability insurance in the amount of $1,000,000 per occurrence, health examinations, income tax withholding and social security payments. Original Certificate of Insurance must be provided prior to acceptance of a physician. Sick leave, vacation, or other benefits are not the responsibility of the Government. At no time shall the contract physician be considered a VA employee for any purpose. Contractor Performance Monitoring shall be accomplished with sign-in/sign-out sheets maintained by a designee of the Director of Specialty Care Healthcare Line (SC HCL) and verified by the Director of SC HCL documenting arrival(s) and departure(s). The Director of SC HCL, Harcharan Singh, M.D., will be the Contracting Officer's Technical Representative and shall monitor performance for contract compliance, knowledge, skills and behavior and immediately notify the Contracting Officer of any noncompliance or unacceptable behavior. Substitution of physician for any reason requires specific authorization by the Contracting Officer, Director of SC HCL, and the Chief of Staff. Any replacement must complete the entire processes listed above. The Government may award a contract based on documentation provided without further discussion. SUBMIT BACKUP DOCUMENTATION WITH OFFER (CV, education, affiliations, experience, at least three (3) recent references names and phone numbers, DEA, Board certification, a listing of relevant continuing education for the last two years, etc). Award will be based on BEST VALUE by the offeror whose quote conforms to the requirements of the solicitation; best meets the minimum needs of the Government, and offers the lowest price. Best value to the Government and veteran shall determine award considering past performance, technical/qualifications, and cost -- considering (1) Prior experience, demonstrated capability and continuity (one physician for the duration of the contract); (2) Past performance/experience and references (preferably VA); and (3) Cost.
