- CLASS CODE: R SUBJECT: Professional, Education Support Services SOLICITATION: F8021111560200 POC: Contact Mary Johnson, Contract Specialist, 202-767-7944, Consuela L Ginn, Contracting Officer, 202-767-7942 DESCRIPTION: This is a combined synopsis/solicitation for non-personal services, to produce a professional training videotape for the Air Force Survivor Assistance Program, to be used by Summary Courts Officer, prepared in accordance with the format in FAR Subpart 12.6 and FAR Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for quote (RFQ). The 11th Contracting Squadron intends to procure solicitation number F8021111560200 as a, Firm Fixed price contract with Cost Line Items for travel and approved miscellaneous expenses. The period of performance will be 15 Jul 01 through 15 Feb 02. The requirements are: CLIN 0001: Scheduling and arranging interviews with 8 members (6 AF member who have served as Family Liaison Officer and two spouses), 1 each; CLIN 0002: Interviewing 8 video participants. 8 each; CLIN 0003: Interviews on camera, 3 days; CLIN 0004: Off-site editing and transcribing of taped interviews (includes review of other footage filmed for previous Survivor Assistance videos), 6 days; CLIN 0005: In-studio editing, 5 days; CLIN 0006: Consultation with AF representatives on duty functions, goals of program, and scripting, 2 days; CLIN 0007: Long distance phone calls, 1 package; CLIN 0008: Estimated travel expenses for 3 round trips, ATL-DCA, lodging and food, private owned vehicle travel, airport parking fees, etc, 1 package. The award date is expected to be not later than 13 Jul 01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-27. The NAICS Code assigned to this project is 611710. The size standard is $5,000,000. This is a small business set-aside. The Contractor shall be responsible for ensuring that all work performed is within the scope defined and the tasks enumerated in the attached Statement of Work. FAR 52.212-1 Instructions to Offerors-Commercial Items (Jun 99) is incorporated by reference and applies to this acquisition. FAR 52.212-2 Evaluation Commercial Items (Jan 99) -- The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers; past performance of the services offered to meet the Government requirement; price. Offerors shall provide backup information supporting the past performance history offered. This could take the form of referral letters or a reliable point of contact for previous jobs. The offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications Commercial Items and DFARS 252.212-7000 Offeror Representations and Certifications with its offer. The FAR Clause 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition and will be incorporated into any resulting contract. Contractors may register in the CCR by visiting the CCR website at www.ccr2000.com . The Government reserves the right to award the contract without initiating discussions. Responses to this solicitation are due by 1000 hrs (10:00 a.m.) on 11 July 2001. An official authorized to bind the company shall sign the proposal.
