- 17. TESTING SPECIALIST. (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Period of performance is 1 Oct 01 ? 30 Sept 02 with two one-year option periods. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number for this request is F22608-01-T-0071 and this is a Request for Quote. (iii) Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. Copies of applicable provisions and/or clauses may be obtained by calling the point of contact listed in this announcement. (iv) This RFQ is set-aside for a small business; the NAICS code is 611710 and the size standard is $5 million. (v) In this solicitation, the estimated requirement is for nine educational sessions per month for the contract period of one year with two one-year option periods. There are three line items in this solicitation and quotes are being requested by ITEM, DESCRIPTION, UNIT PRICE, AND TOTAL PRICE. As required: 0001 Testing Specialist, 108 sessions $______ $______ 1001 Testing Specialist, 108 sessions $______ $______ 2001 Testing Specialist, 108 sessions $______ $______. (vi) Statement of need ? Services non-personal: Provide all labor, tools, parts, materials, facilities, and transportation necessary to perform the services as a test specialist. Session to be defined as the testing of at least five scheduled individuals per session, to include administrative tasks related to these tests. Qualifying tests include, but are not limited to CLEP, DANTES, GED, GRE, GMAT, ASE, ECI, institutionally required admissions tests, civilian institutions ?make up? examinations, and any test required by USAF if not normally administered through the local military test control officer. (vii) Services to be performed at Columbus AFB Education center (building #916) for the period of 1 Oct 01 through 30 Sept 02. Contract is estimated to be 9 sessions per month. (viii) This commercial acquisition incorporates the following clauses and provisions: 52.212-1, Instruction to offerors ? Commercial, Addenda to 52.212-1 Incorporates the statement ?All references to ?offer? and ?offeror? are hereby changed to read ?quote? and ?quoter? respectively,? paragraph (a) The NAIC code is 611710, size standard is $5 million. Quotes will be evaluated according to lowest responsible, responsive quote. 52.212-4, Contract Terms and Conditions ? Commercial Items. Addenda to 52.212-4, Contract Terms and Conditions, incorporate the statement ?All references to ?offer? and ?offeror? are hereby changed to read ?quote? and ?quoter? respectively.? Addenda to paragraph (c) Changes: Add the following sentence ?a unilateral modification for administrative changes such as changes in paying office, appropriation date, etc.? is authorized. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items, applies to this acquisition. All quoters shall include a completed copy of 52.212-3, Offeror Representations and Certification ? Commercial Items, which applies to this acquisition found in 52.252-2, Clauses Incorporated by Reference, states ?As prescribed in 52.107(b), insert the following clause: Clauses Incorporated by Reference (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
