Description

This is a combined synopsis/solicitation for commercial items prepared in=20 accordance=20 with the information in FAR Subpart 12.6 as supplemented with the additional=20 information=20 included in this notice. This announcement constitutes the only solicitation=20 for the required=20 supplies; a written solicitation will not be issued. Paper copies of this=20 solicitation will not be=20 available. The RFQ number is N00140-01-Q-2881 using Simplified Acquisition=20 Procedures=20 under the test program for commercial items found at FAR 13.5. The provisions=20 and clauses=20 incorporated into this solicitation document are those in effect through FAC=20 97-27. It is the=20 contractor's responsibility to be familiar with the applicable clauses and=20 provisions. The clauses=20 may be accessed in full text at these addresses: www.arnet.gov/far and=20 www.dtic.mil/dfars. The=20 NAICS is 325199; size standard of 1,000 employees. FISC Norfolk Detachment=20 Philadelphia=20 intends to acquire through other than full and open competition, IAW FAR=20 6.302-1, ThinPrep=20 Pap Test Packages for the Naval Hospital, Great Lakes. Award of a firm fixed=20 price contract is=20 anticipated. The Government's obligation is contingent upon Congress enacting=20 FY02=20 appropriations. Resultant contract will include a total of 22 units to be=20 delivered commencing=20 with the first week of December 2001 and running through the end of September=20 2002. The=20 contract will include three option periods with additional quantities of 22=20 kits during each of the=20 subsequent three years. The period of performance (delivery schedule) for the=20 base period and=20 each option period requires delivery of one kit on the first Monday of Dec 01,=20 and Mar 02. =20 Delivery of four kits required on the first Monday of May, Jun, Jul, Aug, and=20 Sep 02. If the first=20 Monday of the month is a federal Holiday, delivery shall be made the next day. =20 Offerors are to=20 price the following requirements for the base and each option year. =20 Requirements: CLIN 0001 -=20 ThinPrep Pap Test Packages, with supplies for 500 tests including 500 GYN=20 Transcyt Filters=20 (Clear); 500 Vials of PYT Solution for the ThinPrep Pap Test; 500 ThinPrep=20 Microscope Slides;=20 and 500 Collection Devices configured with P/N 70096-001. CLIN 0002 -- Option=20 I; Same as=20 CLIN 0001; CLIN 0003 -- Option II; and CLIN 0004 -- Option III; Same as CLIN=20 0001. =20 Delivery will be FOB Destination, all transportation charges paid by the=20 contractor to Naval=20 Hospital, Great Lakes. The Government has the unilateral right to exercise the=20 option periods=20 listed above provided that the Government notifies the contractor of the=20 Government's intent to=20 exercise the option within 30 days prior to the expiration of the current=20 period of performance. =20 The following FAR provisions and clauses apply to this solicitation and are incorporated by=20 reference: FAR 52-212-1, "Instructions to Offerors -- Commercial Items"; FAR=20 52.212-2,=20 "Evaluation -- Commercial Items"; 52.212-3 ALT I, "Offeror Representations and=20 Certifications -=20 Commercial Items"; 52.212-4, "Contract Terms and Conditions -- Commercial=20 Items"; 52.212-5,=20 "Contract Terms and Conditions Required to Implement Statutes or Executive=20 Orders -=20 Commercial Items" including (a)(1) and (2) and (b)(1), (5), (11 through 16),=20 (20), (21), (24), and=20 (28i); and FAR 52.217-7 "Option for Increased Quantity -- Separately Priced Line=20 Item: (a) 15=20 days prior to the final delivery under the current CLIN. The following DFARS=20 provisions and=20 clauses apply to this solicitation and are incorporated by reference: =20 252.212-7000, "Offeror=20 Representations and Certifications -- Commercial Items"; and 252.212-7001,=20 "Contract Terms=20 and Conditions Required to Implement Statutes or Executive Orders Applicable to=20 Defense=20 Acquisitions of Commercial Items" including 252.225-7007, 252.225-7012, and=20 252-247-7024. =20 At a minimum, vendors shall supply to this office a signed price proposal on=20 letterhead or a=20 SF1449 for the requested items w/unit price and extended price (same for option=20 pricing),=20 prompt payment terms, remittance address, and a completed copy of FAR 52.212-3=20 ALT I and=20 DFARS 252.212-7000. Offerors will be evaluated using FAR 52.212-2. CBD=20 Numbered Note=20 22 applies to this acquisition, however, all offers received within 15 days (in=20 lieu of 45 days)=20 after date of publication of original synopsis will be considered by the=20 Government. A=20 determination by the Government to not compete this proposed effort on a full=20 and open=20 competitive basis, based upon responses to this notice is solely within the=20 discretion of the=20 Government.
