Description
The CDC has a requirement for a health department web-based software system so that health departments can effectively organize the evaluation data coming from their funded agencies. In 1999, CDC disseminated the document, "Evaluating CDC-Funded Health Departments HIV Prevention Programs: Volumes I & II (CDC Evaluation Guidance, see the website: http://www.cdc.gov/hiv/aboutdhap/perb/hdg.htm)," to 65 health departments that receive funds from CDC to conduct HIV prevention activities. This document provides health departments with guidelines for collecting and reporting evaluation data to CDC. CDC has conducted many training sessions and consultation phone calls to assist health departments with technical assistance needs related to the Evaluation Guidance. Information derived from these training sessions and consultation phone calls suggest that health departments need a data management system that organizes data from their funded agencies to help them comply with the Evaluation Guidance data reporting requirements. CDC anticipates no more than 20 Health Departments that will rely on this web-based software system. Five of them are located on the East Coast, 10 of them are located in the Interior Region and 5 of them are located on the West Coast. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation, 2001-N-00152, is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The associated North American Industrial Classification System (NAICS) code for this procurement is 511210. This requirement is set-aside for small business. Small business size standard: Average annual receipts for each of the three preceding fiscal years do not exceed $18 Million. DESCRIPTION OF SOFTWARE SERVICES TO BE ACQUIRED: Quotes are requested for a web-based data management system that will allow evaluation data, from individually funded agencies of a particular health department, be organized so that the particular health department can aggregate the data efficiently and the aggregate data can be reported to CDC in accordance with CDC's Evaluation Guidance requirements (see http://www.cdc.gov/hiv/aboutdhap/perb/hdg.htm). This system will include the following features: 1) Required data elements (e.g. intervention plan data, process monitoring data) defined in CDC Health Department Evaluation Guidance (Vol 1 & Vol 2, see website above); 2) allow flexibility so that health departments can collect additional data for their own use; 3) Statistical analyses to be performed on data from individual sites for each health department; and, 4) Statistical analyses to be performed and compared with data from multiple sites for each health department. Additionally, this system will be flexible enough to be integrated with existing servers and databases currently used by Health Departments. (See technical requirements listed below in Task 1.) TASKS TO BE PERFORMED: Task 1. The vendor shall meet with the Project Officer in Atlanta to finalize all the requirements related to the software development in order to ensure the product fits the needs of the end users. This meeting should be included as part of your quote. The vendor shall provide the software according to the information indicated above which is defined in the CDC Health Department Evaluation Guidance (refer to website cited above) and comply with all the technical requirements listed below: 1) The system has to be operating under Active Server Pages (ASP); 2) Data queries will allow SQL, Oracle, or DB2; 3) Validation and data quality checks will use Javascript; 4) The key components of the system should allow links for each of the intervention types including the data elements, and tables defined in the CDC Evaluation Guidance; 5) The system needs to have security features to protect the data flow from the funded agencies to their corresponding health departments (some degrees of encryption are required); 6) The system must have at least the following three major components -- I. Data records: each funded agency under the corresponding health departments will be able to use the software system to standardize their data collection. The system will consist of data quality control features including data entry error-check, validation criterion, and controlling skip patterns. II. Date query: each funded agency under the corresponding health departments will be able to use the system to query data gathered (e.g., services provided, material distributed, etc.) in order to measure their own progress. III. Data report: this system will have the capacity to enable each health department to compile cross-site and statewide evaluation. The software must have the flexibility to generate the data efficiently so that Health Departments can submit their required data to the CDC; 7) The system will operate in the health departments' servers which belong to the Windows NT Server operating system with the Internet Information Server (IIS). The system will have a mechanism allowing all the agencies funded by their respective health departments to access this system through the Internet; and, 8) The system will have features that allow health departments to create additional data collection modules or tools. Task 2. The vendor shall install the web-based software into corresponding health departments' current servers (approx. 20) and the corresponding database and provide necessary technical support for the installation of the system (including trouble-shooting the system) to ensure the system is running smoothly in the health department environments. The installation and technical support shall be performed by telephone consultation. CDC anticipates all support issues can be resolved by telephone and vendor shall provide a quotation for this work as though telephone consultation is the mechanism to be used. In the event on-site trouble shooting is required, travel will be funded as a cost-reimbursement line item, set forth as a modification to the purchase order. Travel shall be reimbursed in accordance with FAR clause 31.205-46, Travel costs, and all such costs shall be estimated and approved by the Contracting Officer in advance of the travel. Task 3. The vendor shall provide documentation offering detail of the types of data to be collected by this system and all software applications for this system to CDC and respective health departments. The documentation must be detailed enough so that it will provide the necessary logistic information for those health departments who choose to modify the system for their additional needs. PERIOD OF PERFORMANCE: The period of performance is 45 days from the purchase order effective date. If travel is required, this date may be adjusted. DELIVERABLES: The vendor shall provide to the Project Officer the web-based system and corresponding documentation to the satisfaction of the Project Officer. Task 1 -- Software -- Required 30 days after date of award. Task 2 -- Installation -- Begins after acceptance/approval of Task 1. If by telephone consultation, required within 15 days of approval of Project Officer. If by site visit, required within 15 days of approval by the Contracting Officer. Task 3 -- Documentation -- Required 45 days after date of award. EVALUATION FACTORS FOR AWARD: The FAR provisions 52.212-1, Instructions to Offerors -- Commercial Items, and FAR 52.212-2, Evaluation-Commercial Items apply to this acquisition. The evaluation criteria stated in paragraph (a) of this provision are as follows: 1) technical capability of the software system to meet the requirements stated herein; and 2) price. Also, the vendor shall possess the following minimum qualifications: 1) relevant working experience in designing a web-based evaluation system for health departments; 2) programming experience in ASP within IIS web server and database programming experience in SQL, Oracle, and DB2; 3) demonstration of a good understanding of the requirements based on the CDC Health Department Evaluation Guidance (Vol 1 & Vol 2, see website cited above); and, 4) a history of having produced timely reports, meeting targets, and working with one or more health departments. These factors should be addressed in your quote package. Offerors are advised that if a software licensing agreement is required then it must comport with Federal laws and regulations. If such license agreement conflicts with Federal Laws and/or regulations or otherwise is inconsistent with the spirit and/or intent of Federal commercial item acquisition regulations, then the Government may require wording revision in that license agreement before a purchase order can be awarded. CONTRACT CLAUSES: Offerors shall include a COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS with your quote. The best way to complete this certification is to download the FAR Clauses (Volume II) at http://www.arnet.gov/far, print out the clause 52.212-3, Offeror Representations and Certifications, and fill in the appropriate blanks. This certification must be sent in with your quote. FAR provisions 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition.
