Description

Service contract for Line Item 0001: 1 EA, HEWLETT-PACKARD SONOS 5500 ULTRASOUND SYSTEM MODEL # M2424-25A including 1 EA, DOPPLER PROBE, VCR, and PRINTER. Line Item 0002: 1 EA, 6.2/5.0 MHz OMNIPLANE TEE II PROBE MODEL # 21369A. Full service maintenance agreement for the period 01 Oct 01 through 30 Sep 02, includes unlimited on-site service, telephone assistance, all firmware support, all software upgrades installed as issued by the manufacturer. Also four preventive maintenance/performance inspections per year, labor, parts, travel and shipping expenses necessary to maintain the above instruments in working condition sufficient to meet manufactures specifications. Consulting and instruction review or enhanced level training of system operation will be conducted by mutual agreement between the Contractor and Government Representative. Performance will take place at the Naval Hospital, Camp Lejeune, NC. It is the Governments intent to award a sole source contract to Agilent Technologies, 3000 Minuteman Rd, Andover, MA., the only known responsible source of fulfilling this requirement, however, all responsible sources may submit an offer which will be considered. See Note 22.
