- Contractor shall provide all necessary labor, tools, materials, equipment, parts, maintenance, reports and transportation for home deliveries (including deliveries to those veterans in nursing home facilities) of oxygen delivery system/devices throughout the primary service area of the VA Medical Center, Butler, PA, during the period October 1, 2001 through September 30, 2002, with one (1) option year. All prices quoted must be FOB Destination. Pricing to be all-inclusive, including all mileage, demurrage, visits, and all other supplies, services, and requirements of this solicitation. Contractor must be in an established business of supplying the medical oxygen requirements and must be able to provide twenty-four (24) hours, seven (7) days a week emergency service capability (inclusive of weekend and holidays) in the event that expedited delivery, emergency services, and/or repair are required. The contractor will have personnel on duty or on call, within paging range to respond to calls. All work must be performed in accordance with guidelines established by Federal, State and Local ordinances. The contractor shall permit on-site visits to the contractor's facilities by VA personnel and Joint Commission for the Accreditation of Hospital Organizations (JCAHO) surveyors as needed to assess contracted services, i.e., adequacy, compliance with contract requirements, record keeping, etc. The contractor shall be registered annually with the Food and Drug Administration (FDA) and show evidence of compliance. If the contractor is not JCAHO (or equivalent) accredited, the contractor will have knowledge of and be in compliance with JCAHO standards and must be prepared to provide evidence that it is compliant overall with the JCAHO standards as described in the Comprehensive Accreditation Manual for Home Care. The accredited contractor will maintain their accreditation throughout the duration of the contract and will provide the VA a copy of the certification and following re-certifications during the contract period. If accreditation is lost during the contract period, the contractor will immediately notify the Contracting Officer. The NAICS code for the procurement is 621610. The size standard is $5 million. This procurement has not been set-aside for small businesses but it is offered for full and open competition and is subject to availability of funds. All requests must be in writing. Issue Date on 8/8/01 with opening date of 9/5/01.
