Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is RFQ 541-088-01 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 27. This is a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 421450,, with a size standard of 100 employees). VAMC Cleveland has a requirement for disposable, one time use, Wet Skin Preparation Trays for its' operating room. The skin prep tray shall be sterile and contain the following constituent items: One (1), four compartment tray; One (1), removable two compartment inner tray; Three (3), eight (8) inch sponge sticks (approx 2"x 2" sponge); Two (2), cotton tip applicators (approx 6"); Six (6) wing sponges; One (1), four (4) ounce bottle of providone iodine (cleansing) scrub solution; One (1), three (3) ounce bottle of providone iodine topical (paint) solution, Two (2) blotting towels (approx 18"x22"); Two (2) absorbent (drapes) towels (approx 18" x 16"); and One (1) CSR wrap (Overwrap). For storage and handling, prior to use, the tray shall have a covering to allow storage, handling and transportation in a clean, but not sterile environment. The Wet Skin Prep Tray and the contents of the tray shall be latex-free. Offerors shall include a sample of its Wet Skin Prep Tray with its offer to be received by . Estimated usage is 4200 Wet Skin Prep Trays per year. Orders shall be on a weekly or biweekly basis and made by orally and or in writing. Contractor shall invoice to Fiscal Management Service (04), Louis Stokes Cleveland VAMC, 10000 Brecksville Road, Brecksville, OH 44141. Contract administration is assigned to Contracting Officer, AMMS 90 C (B), Louis Stokes Cleveland VAMC, 10000 Brecksville Road, Brecksville, OH 44141. Contractor shall price separately as follows: 0001 Wet Skin Prep Tray. The Wet Skin Prep Trays will be FOB Warehouse, Louis Stokes Cleveland VA Medical Center. Delivery location is Louis Stokes Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations & Certifications Commercial Items and VAAR 852-219-70, with their offer. The clause FAR 52-212-4, Contract Terms & Conditions -- Commercial Items, applies to this acquisition. The clause FAR 52-212-5, Contract Terms & Conditions Required To Implement Statues or Executive Orders-Commercial Items which shall include the following clauses: 52.203-10, 52.219-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, and 52.225-3 with Alt II. The following standard FAR and VAAR clauses are incorporated and apply to this acquisition: 52.216-18 Ordering (which provides that "Such orders may be issued from September 1, 2001 to August 31, 2002"), 52.216-19 Order limitation Ordering (which provides that for (a) insert 40 each; for (b)(1) insert 100; for (b)(2) & (3) insert N/A; for (d) insert 5 (five)), 52.216-22, Indefinite Quantity (which provides that "the Contractor shall not be required to make any deliveries under this contract after August 31, 2002 unless extended by 52.217-8), 52.217-8 (which provides that the Government may exercise the option by written notice to the contractor within 60 days of the end of the contract), 52.252-2, and, 852.270-1 and 852.270-4. Warranty of said product shall be in accordance with the standard industry practice in effect at time of award. Interested offerors may submit their quotations along with a completed copy of the provision at FAR 52.212.3,VAAR 852.219.70, and a sample of the product on or before 4:30 pm, Friday, August 17, 2001.
