Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures of FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ 652-276 -01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The NAICS is 422690 and the small business size standard is 100 employees. The procurement is a 100% small business set aside. The Hunter Holmes McGuire VA Medical Center, Richmond, VA intends to solicit quotations and award a firm fixed price contract on a competitive basis to the offeror whose quotation meets the requirements of this solicitation. Period of contract will be established October 1, 2000 through September 30, 2002 with four year option periods. Quotations are due by close of business August 13, 2001, local time. All responses should be for specified manufacturer/brand name or equal. Quotations shall be submitted on an "all or none" basis. Partial quotations will be rejected. The Radiology Department of the Hunter Holmes McGuire VA Medical Center has the need for a contractor to furnish the following: 1) Kodak RP developer in 5 gallon increments, premixed and completely tested. Estimated usage is 20 five gallon units per month; 2) HR Simon fixer to fixer tanks. Estimated usage is 20 five gallon units per month. Quotations must include the following: 1) unit cost of each item; 2) descriptive literature and product number for each item offered; 3) Delivery time, F.O.B point, shipping weight and delivery costs. Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting Officer. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and 852.219-70, Veteran Owned Small Business certification must be submitted with offer.
