Description

Provide full coverage Home Oxygen Services to referred and eligible Veterans who receive care at Department of Veterans Affairs (VA) Chalmers P. Wylie Outpatient Clinic, Columbus, OH. Offeror must be able to deliver oxygen, equipment and accessories, within 48 hours after receiving an order, to Veterans living in the following counties in the State of Ohio: Delaware, Fairfield, Franklin, Guernsey, Hardin, Knox, Licking, Marion, Morgan, Morrow, Muskinghum, Perry and Union. Offers will be considered from offerors who are regularly established in the business called for who, in the judgement of VA, are financially responsible and able to show evidence of their reliabilty, ability, experience, equipment and personnel directly employed or supervised by them to render prompt and satisfactory services as called for under the contract. All requests must be mailed using the US Post Office, Federal Express, UPS or other shipping or delivery company, under a company letterhead and must indicate a street address (NO POST OFFICE BOX ADDRESS) in order to send solicitation packages using Federal Express. Solicitation packages will not be ready for mailing until August 13, 2001. THIS SOLICITATION IS A 100% SMALL BUSINESS SET-ASIDE.
