CLASS CODE: R SUBJECT: R Professional, Education Support Services SOLICITATION: F8021111560100-1 POC: Contact Mary Johnson, Contract Specialist, 202-767-7944, Consuela L Ginn, Contracting Officer, 202-767-7942 DESCRIPTION: This notice is being sent to cancel synopsis/solicitation number F8021111560100 issued 29 Jun 01 and resolicit this requirement for the production of a professional training videotape for the Air Force Survivor Assistance Program to be used by the Family Liaison Officer under synopsis/solicitation number F8021111560100-1. Solicitation number F8021111560100 is being canceled in its entirety, due to evaluation factors not being included in the original synopsis/solicitation. Proposals already submitted will be returned per written request. Your written request for return of your proposal and any questions concerning solicitation number F8021111560100-1 shall be received by 1100 hrs (11:00 a.m.) 8 August 01. The new synopsis/solicitation, number F8021111560100-1, is being issued below, and includes detailed information about the requirement and the evaluation criteria that will determine the basis for making award. Solicitation number F8021111560100-1 is a combined synopsis/solicitation for non-personal services, to produce a professional training videotape for the Air Force Survivor Assistance Program, to be used by Family Liaison Officer, prepared in accordance with the format in FAR Subpart 12.6 and FAR Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for quote (RFQ). The 11th Contracting Squadron intends to procure solicitation number F8021111560100-1 as a, Firm Fixed price contract with Cost Line Items for travel and approved miscellaneous expenses. The period of performance will be 3 September 01 through 1 May 02. The Contractor shall be responsible for addressing all the tasks identified in the attached Statement of Work (SOW) in their proposal, while ensuring that their proposal is written in accordance with the SOW. The requirements are: CLIN 0001: Scheduling and arranging interviews with 8 members (6 AF member who have served as Family Liaison Officer and two spouses), 1 each; CLIN 0002: Interviewing 8 video participants. 8 each; CLIN 0003: Interviews on camera, 3 days; CLIN 0004: Off-site editing and transcribing of taped interviews (includes review of other footage filmed for previous Survivor Assistance videos), 6 days; CLIN 0005: In-studio editing, 5 days; CLIN 0006: Consultation with AF representatives on duty functions, goals of program, and scripting, 2 days; CLIN 0007: Long distance phone calls, 1 package; CLIN 0008: Estimated travel expenses for 3 round trips, lodging and food, private owned vehicle travel, airport parking fees, etc, 1 package. The award date is expected to be not later than 31 August 01. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-27. The NAICS Code assigned to this project is 611710. The size standard is $5,000,000. This is a small business set-aside. This acquisition is being procured as a competitive Best Value award in accordance with Air Force Federal Acquisition Regulation Supplement (AFFARS) 5315.3, Source Selection, considering reasonably provided information. The Government will award a contract resulting from this solicitation to the most responsible offeror whose offer, conforming to this solicitation, proves to be the most advantageous to the Government, based upon an integrated assessment of the following evaluation factors, listed in order of importance: 1. Mission Capability, 2. Past Performance, and 3. Cost/Price. Evaluation factors other than cost or price, when combined are significantly more important than cost or price; however, cost/price will contribute substantially to the selection decision. The Government seeks to award to the offeror who gives the Air Force the greatest confidence that it will best meet or exceed the requirements affordably. This process permits tradeoffs among cost/price and non-cost factors, ultimately allowing the Government to accept other than the lowest priced proposal. The perceived benefits of the higher priced proposal shall merit the additional cost.=20 The contractor must submit a technical proposal with resume that demonstrates how they will meet the requirements in paragraphs a. and b. of Mission Capability Subfactor 1: Production Capability. (1) Mission Capability (Within the Mission Capability factor, the subfactors are listed in descending order of importance.) (RATINGS: Acceptable/Unacceptable) Each proposal will be compared to the requirements identified in this synopsis/solicitation and the Statement of Work for a determination of adherence to all requirements and understanding of the effort required. Subfactor 1: Production Capability a. Ability to Satisfy Statement of Work (SOW) -- Serve as the overall production manager and producer of a 30-minute video to be used for training purposes as part of the Air Force Survivor Assistance Program (AFSAP) and Mortuary Affairs. Contractor shall produce a video which educates the "lay people" on the dynamics of the loss of a loved one and the diversity of potential reactions. The Contractor shall cover each of the following areas in the video: a. The various organizations involved in briefing NOK/families on AF benefits and entitlements. b. The importance of accurate, timely inventory of items and return to family. Must clearly understand need to document any items destroyed IAW the applicable AFI, etc. c. Teaching the audience on what and what not to say to surviving family members and to recognize the needs of family members of all ages. d. That no FLO experience is like another, different circumstances and challenges will exist. FLO must devote time to solve the issues in the best interest of the family, not necessarily in the most expedient manner. e. Emphasize the importance of coordinating actions with the legal community. f. Emphasize importance of documentation of all actions taken as FLO g. Emphasize that FLO duties may be very simple or very complex, but requires full attention until complete, regardless. -- Prepare questions related to the above required topics for review, and possible addition, by the government. Conduct interviews with at least 8 AF Family Liaison Officers (FLOs) and/or spouses (appropriate mix) based on these questions for inclusion in the video -- Scheduling, arranging, and conducting both on and off-camera (in-person and telephone) interviews with spouses of deceased AF members and personnel who previously performed duties as a FLO, completing off-site and in-studio editing and transcribing of taped interviews, and consultation with AF representatives. -- Review AF video footage from previous AFSAP videos for applicable insertion into the video. -- Studio edit and script the content of the video from the interviews and previous AFSAP footage into a 30-minute training video (must be between 25 and 40 minutes). -- Provide receipts for all travel-related expenses and proof of long distance telephone calls for reimbursement. b. Submission of Resume that meets or exceeds the experience requirements: -- Mental health professional, such as a certified grief counselor, social worker, psychotherapist or psychologist with extensive education and background in training/counseling individuals to deal with the psychological/physical/emotional consequences of sudden and unexpected loss of life.=20 -- Experience dealing with survivors of individuals killed in aircraft mishaps is preferred.=20 -- Prior knowledge of and experience with the AFSAP and mortuary affairs is highly preferred. -- Knowledge of investigative boards, processes, and information flow as is pertains to aircraft mishaps, terrorist attacks, suicides, homicides, etc and how can affect survivors acceptance of their loss is preferred. Subfactor 2: Sample Training Video a. Demonstrates experience producing training videos as described in the SOW, i.e., using real people to share actual experiences. -Provide sample training video company has produced. Training videos as described above are different than a marketing type video -- selling an item or program with "actors" reading from a script. Sample video must illustrate ability to use real people to share their experiences in a way others can learn from them. (2) Past Performance (Ratings: In accordance with AFFARS 5315.305(A)(2)(S-92) -- Prior experience with the AFSAP and/or AF mortuary affairs program is highly preferred. Offerors are to complete and return the attached Past Performance Information Sheet to this office as part of their proposal for 5 recent (=3Dwithin 5 years) and relevant (=3Dsame or similar product) contracts. Offerors without a record of relevant past performance or for whom information on past performance is not available will not be evaluated favorably or unfavorably on past performance. As a result, a Neutral/Unknown Confidence Rating would be received. (3) Cost/Price-Affordable, Realistic, and Reasonable Unrealistically low or high proposed costs/prices may be grounds for eliminating a proposal from competition on the basis that the offeror either does not understand the requirement or has made an unrealistic proposal. The offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications- Commercial Items and DFARS 252.212-7000 Offeror Representations and Certifications with its offer. FAR 52.212-1 Instructions to Offerors-Commercial Items (Jun 99) is incorporated by reference and applies to this acquisition. FAR 52.212-2 Evaluation Commercial Items (Jan 99). The FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition and will be incorporated into any resulting contract.
