Description

This solicitation for commercial items is prepared in accordance with FAR Part 13 and the format in FAR Subpart 12.303, as supplemented with additional information included in this notice. This announcement constitutes the official notification for which quotes are being requested and a written solicitation will be issued. The solicitation document Request for Quotes (RFQ) 187-013-1 and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation Circular 97-27. This procurement is being advertised as a fully competed procurement, under North American Industry Classification System (NAICS) code 621111 with a size standard of $5.0 million. The Federal Bureau of Prisons, Federal Correctional Institution (FCI), Phoenix, AZ intends to award a contract for Cardiology services to the contractor who submits the best value offer (a) within the City of Phoenix, County of Maricopa. The services required include but are not limited to examination, evaluation, and recommendation of treatment to FCI health services staff. Contractor will provide written and/or dictated reports within five (5) working days after evaluation on all patients referred for evaluation, and will include recommendations for further diagnostic testing and/or treatment. Services will be required from 1 October 2001 through 30 September 2002 with four (4) twelve-month option periods, for an estimated twelve (12) four-hour sessions required during a twelve-month period.
