- The Federal Bureau of Prisons, Western Regional Office, is seeking multiple qualified sources capable of providing medical services. The medical facility shall be responsible for furnishing all necessary professional health care services, including but not limited to diagnosis, consultations, treatment, reporting, management oversight and training. The professional services required include Inpatient/Outpatient Facility Services, Inpatient /Outpatient Physician Services and Outpatient Institution Services. These services shall include but will not be limited to Ophthalmology, Orthopedics, Dermatology, Otorhinolarynogology, Radiology, Infectious Disease, Geriatrics, Dialysis, Anesthesiology, Detoxification, Pulmonary Medicine, Outpatient Physical Therapy, Psychiatry, Carbology, Urology, General Surgery, Gastroenterology, Endocrinology, Internal Medicine, Chemotherapy, Cardiovascular Surgery, Vascular Surgery, Nuclear Medicine, Plastic and Reconstructive Surgery, Radiation Therapy, Oral Surgery, Laboratory Services, Professional Services, Telehealth Medical, Neurosurgery, Anesthesiology, Podiatry, ENT Specialist, Optometrist and Neurology. Location of the medical facility shall be within fifty miles of the Federal Correctional Institution, 5701 8th Street, Camp Parks, Dublin, CA 94568. The Federal Bureau of Prisons anticipates the award of a Performance Based Firm-Fixed priced contract resulting from this Request for Proposal. The estimated quantities are as follows for the Inpatient/Outpatient Facility Services and the Inpatient/Outpatient Physician Services; Base Year: Inpatient Facility/Physician Services, 182 days, Outpatient Facility/Physician Services 300 visits, Option Year #1, Inpatient Facility/Physician Services, 191 days, Outpatient Facility/Physician Services 315 visits, Option Year #2, Inpatient Facility/Physician Services 200 days, Outpatient Facility/Physician Services, 331 visits, Option Year #3, Inpatient Facility/Physician Services 210 days, Outpatient Facility/Physician Services, 348 visits, Option Year #4, Inpatient Facility/Physician Services 221 days, Outpatient Facility/Physician Services, 365 visits. The Outpatient Institution Services will vary according to the Specialist required; Orthopedic Surgeon -26 visits per year, Dermatologist -- 4 visits per year, Psychiatrist -- 144 visits per year, Neurologist -- 16 visits per year, Urologist -- 4 visits per year, General Surgeon -- 24 visits per year, ENT Specialist -- 4 visits per year, Oral Surgeon 12 visits per year, Optometrist -- 48 visits per year. Each session shall consist of four hours. The above cited requirements are estimates only, and are not a representation to the Offerors that the quantities will be required or ordered. The anticipated period of contract performance will be; Base Year: Date of Award through 12 months from award date; Option Year #1: 13 months from date of award through 24 months from date of award; Option Year #2; 25 months from date of award through 36 months from award date; Option year #3; 37 months from date of award though 48 months from award date; Option Year #4; 49 months from date of through 60 months from award date. Proposal packages will be approximately available 11/01/01 with an approximate opening date of 12/07/01. All offers received in response to this notice, will be subject to the evaluation criteria as established in the Request for Proposal.
