17. Description (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request For Quotation, # F5HOSP11650800. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. (iv) Notice: This solicitation is a 100% set-aside for small business concerns. The North American Industry Classification is 811213? Hospital Communication Maintenance. The size standard is $5,000,000. (v) Items to be Procured: Require contractor to provide all personnel, equipment, tools, materials, supervision, and other items necessary to perform installation and PBX reprogramming of the Telephone Communication/Nurse Call System, Emergency Response Alarm System, Radio (Stereo), Tape System, and Public Address System as in accordance with the Performance Work Statement and Equipment Inventory. All information for the subject solicitation is available at www.eps.gov, no paper solicitations or specifications will be made available. L/I 01: Monthly Service for telephones and all other associated communication systems. L/I 02: PBX Reprogramming, Equipment Moves, and Wiring. (vi) There is no additional description for the preceding line item. (vii) Performance period shall be from 1 Oct 01 to 30 Sep 01. Place of performance shall be 5th Medical Group, 10 Missile Avenue, Minot AFB, ND 58705; FOB Destination. (ix) Evaluation will be based on price and price related factors.
