Description

RFP 659-3-02, Amendment No. 2 is issued to add the following: Imaging services at the Charlotte CBOC are required within a five (5) mile radius of the 5516 Central Ave., Charlotte, NC address. The contractor shall agree to the VA's Reasonable and Customary rate in place at the date of service. Offers are encouraged to offer a discount of the VA's Reasonable and Customary Rate. There are seven (7) contract line item numbers (CLIN) per year (CLIN 1, 2, 3). The CLIN for each year will reflect (1) Cranium, including sinue, mastoid, jaw, etc, est 6/yr @ $________; (2) Chest-single view est., 1/yr @ $__________; (3) Chest multiple view est. 378/yr @ $_________; (4)Skeletal-Bone and Joints est. 258 yr @$________; (5) Abdomen-Kub est 12/yr @$_______; (6) Obstructive series est. 9/yr @$______ and (7) Skeletal-spine & scaroiliac est 219/yr@$________. The quantities provided are estimated based on past history and represent one year. Firms are reminded when preparing and submitting their proposal this estimate is for one year. However, the quote submitted must reflect pricing for one year, plus two (2) one-year renewal options. The VA does not guarantee any specific number of exams nor that all listed exams will be ordered. Award will be based on lowest price and submission of required documents in the Statement of Work. The date for acceptance of offers is extended to August 17, 2001.
