Description

The Utah National Guard is issuing a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are hereby requested. A written solicitation will not be issued. This is issued as a Request for Quote (RFQ), Number DAHA42-01-T-0007, which incorporates provisions and clauses in effect through Federal Acquisition Circular 97-27. This requirement will be 100% small business set aside. The North American Industry Classification System (NAICS) Code will be 337214, size standard is 500 employees. DESCRIPTION: Design, furnish and install modular furniture for 14 offices, 8 ft by 8 ft., a common printer/copier area, and a common breakroom area. A Pre-Proposal Conference will be held on August 16, 2001 at 10:00 A.M. local time at the DOL area of the USPFO Warehouse, 12953 South Minuteman Drive, Draper, UT. A floor plan of the area and offices will be provided at the conference. Agency requirements will be discussed and industry feedback is encouraged. It is strongly suggested that anyone wishing to submit an offer attend this conference. Offerors will then submit a rough design and cost proposal to include descriptive literature of the product being proposed. EVALUATION FACTORS: The contract type for this procurement will be fixed price and award will be made on the basis of best value to the Government, price and other factors considered. Selection will be based on the following evaluation factors, in order of importance: (1) technical factors, to include a proposed design which meets the criteria discussed at the pre-proposal conference, the offerors capability to provide the items required, and the quality of the proposed item; (2) total cost or price. INSTRUCTIONS: To receive award, the offeror must be registered in the Central Contractor Registration (CCR) database which can be accessed at web site http://www.ccr2000.com or call CCR Assistance Center toll free at 1-888-227-2423. Offers shall include (1) a cost proposal and rough design, (2) descriptive literature of product proposed (3) FAR 52.212-3, Representations and Certifications -- Commercial Items, filled in completely, and (4) a list of at least three customers to which the offeror provided similar items and services as covered by this solicitation within the past three years. Offers shall be submitted NLT 4:00 P.M. local time on August 30, 2001 to USPFO for Utah, Attention Sherri Shepherd, 12953 South Minuteman Drive, P.O. Box 2000, Draper, Utah 84020-2000. Facsimile offers will be not be accepted. All offers must arrive at the place and by the time specified.
