Description

This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation. Written quotations will not be issued and each contractor is expected to respond on his/her letterhead with all required product information. RFQ 621-01-0155 is hereby assigned to this procurement and shall be referenced on all correspondence relating to this acquisition. This RFQ is issued using Simplified Acquisition Procedures and incorporates all provisions and clauses as those in effect through FAC 97-27 and VAAR 97-1. The James H. Quillen VAMC, Mountain Home, TN 37684 has a requirement to purchase one (1) IMAGEnet 200 Telemedicine System with TRC-NW6S Package which includes: TOPCON TRC-NW6S Digital Non Mydriatic Retinal Camera, Field Changer 30 & 45 Degree, Video Relay Lens, Sony DXC-390 3 Chip Video Camera; Compaq PIII 733 Mhz Computer System, 17" SVGA Menu Monitor and 16 Mbyte Video Adaptor, SVGA Imaging Board, 10 Gigabyte Hard Disk Drive with 128 Mbyte RAM, 48X Internal CD ROM, CD Archiving 16X Write/40X Read, Topcon Telemedicine Software Package, MS Windows NT Software Package, Norton Anti-Virus, Carbon Copy, Mosaic Software Package, Epson Color Printer for Results Output, US Robotics 56 Kps Modem for System Support, and one (1) Work List Modality Module (for IFA or Vista) BRAND NAME OR EQUAL. Pricing to include shipping costs, installation and training. The delivery date will be no later than 30 days after award of a contract. Contract award is subject to the availability of funds.
