Description

This is a notice of the intended issuance of Request for Proposal (RFP) 648-97-01 for Assisted Living Services. The RFP is in a commercial format and the package will be available on or about 17 August 2001. Multiple Award Indefinite Delivery Contracts will be issued for requirements within defined geographical areas. As part of the Veterans Healthcare Administration's Assisted Living Pilot Program, the Portland Department of Veterans Affairs located at 3710 SW Vets Hospital Road, Portland, OR 97201, is seeking vendors/contractors to provide contracted Assisted Living services to eligible veterans in the following counties: Oregon: Benton, Clackamas, Clatsop, Columbia, Crook, Deschutes, Hood River, Jefferson, Lincoln, Linn, Marion, Multnomah, Polk, Sherman, Tillamook, Wasco, Washington, Wheeler, Yamhill, and Clark; Washington: Cowlitz, Klickitat, Pacific, Skamania, and Wahkiakum, Applicants must be those who are licensed providers of Assisted Living Services in Assisted Living Facilities. All facilities wishing to participate in this program must accept Medicaid. There will be a meeting for vendor applicants wishing to participate. This meeting is crucial to the ultimate success of this pilot program and will be held at the Portland VA Medical Center, Building #101, Room 201, 3710 S.W. Vets. Hosp. Road, Portland, OR 97201, on August 24, 2001, at 9:00 to 11:30 AM. Questions about the Assisted Living Pilot Program and the contracting process will be addressed at that time.
