Description

The Carl T. Hayden VA Medical Center, 650 E. Indian School Road, Phoenix, AZ 85012 anticipates contracting for general dentistry services for eligible veterans. Services are to be performed in the contractor's office and must be located in Maricopa County, Arizona. Quotes will be accepted only from offerors who are financially responsible. All responsible offerors may submit an offer and will be considered. It is the intention of the VA to award multiple contracts. The NAICSC code is 621210, size standard $5.0 applies. The contract base year is anticipated from October 1, 2001 to September 30, 2002, with four (4), one (1) year renewal options. This is a total 100% small business set-aside. Solicitation documents will be available on or about September 4, 2001. All requests for copies of the solicitation shall be in writing. Telefax requests shall be accepted at (602) 212-2155. NO TELEPHONE requests shall be accepted. Solicitation is issued as a Commercial Item format per FAR Part 13.5.
