Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as RFP 688-16-02. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-23. The NAIC code for this solicitation is 811310. The small business size standard is $5.0 million. This solicitation is a small business set-aside Contractor shall furnish all necessary labor, travel, materials, equipment, etc., necessary to provide full maintenance service and one (1) preventive maintenance inspection per year and emergency repairs for the following Government-owned equipment listed in accordance will all terms and conditions of this solicitation for the period of 10/1/01 thru 9/30/02 with two one-year renewal options subject to the availability of funds. Work shall include complete ANSI calibration which includes measurement and, if necessary, adjustment to appropriate standard of frequency, distortions, sound pressure level, attennator linearity and crosstalk, as well as a complete mechanical inspection. ABR units will be Stimulus checked. Parameters which are not specifically mentioned, but which are part of the manufacturers' specifications will also be checked and adjusted as necessary. A certificate of calibration will be mailed after each calibration. Preventive maintenance: Calibration also includes procedures such as checks (and replacement as needed) of cords and cushions; cleaning of contacts, connectors and switches; and inspection of overall condition. Emergency Service: Unlimited emergency repairs with two-hour telephone and 8 hour physical response (as necessary on normal workdays). PARTS: Parts with an individual list price of $25.00 or less will be replaced when necessary by the contractor. Parts over $25.00 must be pre-authorized, then billed separately.HOURS OF COVERAGE: Normal workings hours are Monday thru Friday 8:am -- 4:30pm, excluding federal holidays. PRICING: Base Year: 10/1/01-9/30/02 $___________per month x 12 months @ $_________per year, Option Year 1 -10/1/02-9/30/03 $_________per month x 12 months @_________per year, Option Year II 10/1/03-9/30/04 $___________per month x 12 months @ _______________per year.
