- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as RFP 688-14-02. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-23. The NAIC code for this solicitation is 811310. The small business size standard is $5.0 million. This solicitation is a small business set-aside. Contractor shall provide all labor, equipment and materials necessary for the maintenance and repair of automatic doors for the Department of Veterans Affairs Medical Center, 50 Irving Street NW, Washington, DC for the period of 10/1/01 thru 9/30/02 with two one-year renewal options, subject to the availability of funds. Contractor shall perform a complete preventive maintenance procedure on all doors indicated each quarter, which will be scheduled with the COTR (Contracting Officers' Technical Representative). The work shall consist of: (a) Visually inspect each door assembly for obvious problems, malfunctions and broken parts. (b) Remove cover of operating mechanism to reveal control panel. (c) Check and adjust doors for proper operation of: opening speed, backlatch operation and speed, closing speed, latching operation and speed. (d) check and adjust all actuating and safety devices, if so equipped such as: proximity or electronic actuating devices, proximity or electronic safety devices, time delay devices and wall or door actuating devices. (e) Check all belts, cables, rollers, sprockets, and all door and other moving parts for proper alignment, lubrication, and operation. (f) Ensure all bolts, nuts, screws, etc., properly tightened and secured to prevent future loosening.(g) Replace all defective components and parts to guarantee continuous and proper operation. (h) Perform final operating inspection (i) Re-install all covers properly and securely (j) Clean up door area as may be required to maintain proper housekkeeping. Dispose of worn out or non-operating parts off site as may be required. (k) Return any parts to manufacturer as may be required as trade-ins or in-warranty failures (l) Record all results, findings, action taken, and replacement parts used. Record travel time, hours on-site, and action taken on a customer service report (m) Equipment serviced must be clearly identified by name, PMI number or Inventory number (n) Provide one copy of the service report to the Engineering Office-Mechanical Equipment Room #1) 2. Provide emergency response for repairs to individual doors within (24) hours of notification, excluding holidays, weekends and nights. This service will include parts, materials, equipment, tools and labor to effect repairs. Vendor shall have access to replacement parts within (24) hours of determination of need. Repairs shall be made the same day as the service or provide a schedule of repair. All repair are to be completed within 48 hours of the first site visit. 3. Door glass, headers, and parts damaged by other than normal wear are not included in this contract. Contractor Experience: Successful offeror shall have an established business and the business must have a minimum of two (2) years of successful experience in the field of providing maintenance and repair service for automatic doors. 5. Competency of Personnel Servicing Equipment: a. The Field Service Engineer (FSE) must be authorized by the contractor to perform maintenance services. The Contractor shall provide written assurance of the competency of their personnel and list of the credentials of approved FSEs for each model of equipment serviced by the contract at the VA. B. Each respondent must have an established business with an office and full time staff. The staff includes a "fully qualified" FSE and backup. "Fully qualified is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized training program, for the equipment identified in the schedule. For field experience, the FSE(s) has a minimum of two years of experience, with respect to scheduled and unscheduled preventive and remedial maintenance, on the listed equipment. C. If subcontractor(s) are used, they must be approved by the CO/COTR. The contractor shall submit to the CO any intentions and or changes regarding the use of subcontractors. 6. Hours of Coverage: Normal VAMC working hours are Monday-Friday,8AM 4:30PM excluding federal holidays. All service/repairs shall be performed during normal hours of coverage with prior approval of the COTR. At the request of the FSE, the COTR may approve service outside normal hours of coverage at no extra cost to the government. 7. Parts: a. The Contractor shall furnish and replace all equipment replacement parts. These parts are included in the price of this contract. The Contractor shall have ready access to all equipment parts, including unique and high morality replacement parts, in order to minimize downtime. B. Only new standard parts (manufactured by the maker of the equipment or equal thereto) shall be furnished by the Contractor. Rebuilt parts, used parts, or those removed from the same model of equipment shall not be installed without prior approval of the COTR. All parts supplied by the Contractor shall be of current manufacture and have complete versatility with the presently installed equipment. All parts shall perform identically to the original equipment specifications. PRICING SCHEDULE: Base Year $________per month x 12 months @ $__________year, Option Year 1 $___________per x 12 months @ $__________per year, Option Year II $__________per month x 12 months @ $__________ year.
