Description

1. REPLACES NOTICE FROM 8/8/01. 2. The VISN 22 Network Business Center seeks to establish a firm-fixed price contract under the simplified acquisition procedure for the provision of Preventive Maintenance and Emergency repairs on Government-owned radiology equipment. The equipment is Fuji, Gendex, Agfa Film Processing/Laser Imager equipment, Konica Xomat Film Processors and Laser Printers. Included are the chemical solutions, fixer developers and the Electrolytic Silver recovery. The equipment is located at the following VA facilities: the Jerry L. Pettis Memorial in Loma Linda, CA, Long Beach, San Diego CA, West Los Angeles, BAKERSFIELD OPC and the Sepulveda Ambulatory Care Centers, CA. The equipment is to be maintained by providing all necessary labor, materials, tools, parts, travel, shipping expenses and non-expandable parts to provide unlimited emergency repair service and preventive maintenance inspections. Contractor shall provide manuals and schematic drawings to perform scheduled maintenance and repair service (including Cadi tanks) to keep equipment in first-class operating condition. The resulting contract shall be effective for one (1) base year with the government having the option to exercise four (4) additional option years. Maintenance shall be performed during normal business hours 8:00 am to 5:00 pm., Monday through Friday, excluding legal Holidays. With the exception of one (1) machine at Loma Linda (Mammography) requiring daily PMI, all PMI's are scheduled to be accomplished monthly and Bi-weekly. Prospective contractor shall submit credentials, training certificates and certification with proposals. Servicing technicians must be certified technical representatives capable of keeping the equipment up and operational according to manufacturers' standards. The provisions of FAR 52.212-1, Insructions to Offerors -- Commercial Items Applies. Offerors are required to complete and include a copy of the following provisions with their proposals: FAR 52.213-3 , "Offeror Representation and Certification -Commercial Items" and FAR 52.212-4, "Contract Terms and Conditions-Commercial Items". The following Addendum Clauses "FAR 52.218, "Availability of Funds", FAR. 52.212-5, "Contract Terms and Conditions Required to Implement Statutes or Executive Orders", FAR 222-26, Equal Opportunity", FAR 52.222-35 "Affirmative Action For Special Disabled and Vietnam Era Veterans", FAR 52.222-36, "Affirmative Action Workers", FAR 52.222, FAR.52. 222-37 "Employment Reports on Special Disabled Veterans and Veterans of Vietnam Era", Provisions of FAR 52.212-2, "Evaluations of Commercial Items apply". Award will be made to offeror that meets solicitation's minimum criteria for technical acceptability at the lowest price to be dertermined in the best interest of the government. The offeror must provide references from at least three (3) current accounts where the contractor has provided the same or similar services on the equipment mentioned herein and/or evidence of certification as an authorized factory trained facility.
