- Description
- (I) This is a combined synopsis/solicitation for commercial services prepared in the format in the F.A.R., Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation. NO OTHER DOCUMENTS WILL BE ISSUED. (II) This solicitation is issued as Request for Quotation (RFQ) 40804-2B140014. (III) This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 97-27, dated August 7, 2001. (IV) This solicitation is set-aside 100% small business. The North American Industry Classification Systems (NAICS) code is 621399. (V) A -- Service to be performed: Dental Hygienist duties at the US Penitentiary/Federal Prison Camp, Leavenworth KS. B -- Statement of Work: Contractor will take and develop intra- and extra-oral radiographs; must operate panoramic machine. Will measure and chart depth of periodontal pockets. Perform CPITN screening examination. Monitor inmates with significant periodontal disease. Provide dental prophylaxis, deep root planing and curettage. Evaluate inmate's medical history prior to treatment. Serves as institution's coordinator for preventive dentistry program. Provide oral hygiene instruction and oral health eduction to inmates. Apply desensitizing agents. Give fluoride treatments. Provide chemical and local antibiotic therapy for periodontal disease. Apply pit and fissure sealants. Retrieve patient's medical records for dental documentation. Make necessary entries into chart. Maintain the dental station in a clean, orderly and good operating condition. Sort and store instruments, medications, linens, and other supplies. Maintain supply levels; oil and clean equipment; clean and sterilize instruments; sharpen instruments; dispose of soiled or used materials. Report any defective or broken equipment to Chief Dental Officer. Maintain proper infection control in radiographic techniques. Sterilize contaminated biological waste as approved the the Chief Dental Officer. C -- Work Hours: Six (6) sessions per week, each session will be three (3) hours in length. Sessions to be performed during the hours between 7:30am and 4:00pm, Monday thru Friday, as mutually agreed upon between contractor and Chief Dental Officer. D -- Supplies: All basic supplies and lab coats will be provided by the government. Lab coats will NOT be worn out of the institution. E -- Qualifications: Contractor must have two (2) years dental experience. F -- Credentials: Contractor must be licensed by any state licensing/accreditation authority. G -- Institution Security and Inmate Management: Contractor will monitor the attendance of inmates who are placed on the call out sheet for care and treatment. Infractions will be reported to Chief Dental Officer. Contractor must have a clear background check PRIOR to contract award. Prior to reporting to duty in the Dental Office, contractor must attend an orientation class; reimbursement for the time in class will be based on the contract hourly rate. (VI) Evaluation Criteria: Offers received will be evaluated on (1)Technical/dental experience, (2) Experience in a correctional environment, and (3) Price, with #1 being the highest evaluated factor. The contract may be awarded without discussions, therefore, offerors are instructed to submit their best offers initially. (VII) Performance Period: Date of Award thru 9/30/2002, with two (2) option periods to follow, for a total contract period not to exceed three (3) years. THIS IS NOT A PERSONNEL APPOINTMENT. The award of the contract does not constitute an employee/employer relationship. Contractor will not be supervised, but will be monitored to determine if services rendered are in compliance with the contract terms and conditions. Payment will be made upon receipt of an invoice, and after verification that contract terms and conditions have been met. (VIII) FAR provision 52.212-1, Instructions to Offerors-Commercial Items (Oct 2000), is referenced. The following addenda are included by reference: At paragraph C, Period of Acceptance of Offers, offerors must agree to hold the prices submitted for sixty (60) calendar days from the date specified for receipt of offers. At paragraph H, Multiple Awards, the Contracting Officer has determined multiple contracts are NOT applicable in this solicitation, and the pargraph is hereby removed in its entirety. (IX) FAR provision 52.212-3, Offeror Representations and Certifications-Commerical Items will be completed prior to award. (X) FAR provision 52.232-33, Mandatory Information for Electronic Funds Transfer Payment applies to this solicitation. In addition, contractor MUST provide federal tax identification number or social security number PRIOR to award. (XI) Offers will NOT be accepted by fax. Offers must be received in the Contracting Officer no later than 2:00pm, Central Time, October 3, 2001. Envelopes must be clearly marked with the solicitation number. THIS IS NOT A SEALED BID. Late offers will be rejected, unopened.
