- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotation are being requested and a written solicitation will not be issued. The solicitation number is RFQ 541-102-02 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 27. This is an unrestricted procurement. The North American Industry Classification System (NAICS) code is 811219, with a size standard of $5M). Louis Stokes Cleveland VAMC has a requirement for a maintenance and repair service agreement for a TECNAI 10 Electron Microscope and Camera System located at room E14, Louis Stokes Cleveland VAMC, Wade Park Unit, 10701 East Blvd., Cleveland, OH 44106. The contract shall be for one (1) year commencing October 1, 2001 with two (2) one year option renewals. Item 0001 Maintenance and Service Agreement for the TECNAI 10 Electron Microscope and associated Camera System. Included are labor and transportation. All parts are included except for Adjusted apertures (including Whenelt apertures), Anti-contaminations devices, Backscatter detector chips, Binoculars, Cooling blades and cooling traps, Cranes and lifting devices, FEG filaments, Dark-filed detector chips, Flat firm holders and inserts, LaB6 filaments, Lens housings, Plate Camera load boxes, Specimen holders, Tools and tool kits, Wehnelt assemblies, Air Compressors, Aperture holders, Bright-field detector Scintillators, Camera (including Polaroid, 35,mm), cosmetic damage (including table tops0, Dewar vessels, Desiccators, Fiber optica, Fluorescent viewing screens, Leaded Glass (including viewing windows), Manuals, Plate camera unloaded boxes, Specimen loading and storing boxes, Water chillers (except Zephyr). Replacement parts shall be authorized for the equipment. Emergency response time is 48 hours. Service coverage is Monday through Friday 8:00 a.m. to 4:30 p.m. The successful offeror shall demonstrate that it can obtain the authorized parts and qualified to perform the authorized maintenance and service in accordance to the manufacturer's specifications. Warranty of said product shall be in accordance with the standard industry practice in effect at time of award. Contractor shall submit invoices to Fiscal Service (04), Louis Stokes Cleveland VAMC, 10000 Brecksville Road, Brecksville, OH 44141. The contractor shall price base and each option year separately. Service shall be performed at room E14, Wade Park Unit, Louis Stokes Cleveland VA Medical Center, 10701 East Blvd, Cleveland, OH 44106. The following provisions and clauses apply to this acquisition: 52.212.-1, Instructions to Offerors-Commercial Items applies to this acquisition with the following paragraphs, (e) and (h), are deleted; 52.212-3, Offeror Representations & Certifications Commercial Items; FAR 52-212-4, Contract Terms & Conditions -- Commercial Items, FAR 52-212-5, Contract Terms & Conditions Required To Implement Statues or Executive Orders-Commercial Items which shall include the following clauses: 52.222-3, 52.233-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34, 52.222-41, 52.222-42, 52.222-43, and 52.215-1 Instructions to Offerors-Competitive Acquisition; and 852.219-70 Veteran-Owned Small Business; 852-270-4 Commercial Advertising; 52.215-5 Facsimile Proposals; 52.232-18 Availability of Funds and 52.232-19 Availability of Funds for Next Fiscal Year; 52-217.8, Option to Extend Services (with the following blank filled in option by written notice to the Contractor within "60 days"). 52-217.9, Option to Extend the Term of the Contract (with the following blank filled in: (a) "10 days" and "60 days" and (c) "three (3) years six (6) months"). Offerors responding to this announcement shall provide the information required in provisions 52.212-1, a completed copy of 52.212-3 and certification required by 852.219-70. Also, written evidence that it is authorized to perform support and maintenance on the Tecnai 10 and camera, and written evidence that it is able to obtain authorized parts for maintenace and support. Quotations shall be received by Monday, September 10, 2001 no later than 4:30 p.m. local time.
