This is a combined/synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation N00259-01-T-0043 is issued as request for quotations (RFQ). Award will be made on PRICE, TECHNICAL EVALUATION OF ALTERNATE SOURCES, DELIVERY and PAST HISTORY. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-27. This is an FY2001 STANDING ORDER for RADIOLOGICAL SUPPLIES/ PRODUCT CALLED SYRINGE, POWER INJECTOR 50/CASE, CATALOG # CTP-200-FLS, MANFACTURED BY: "MEDRAD INC.." This award will be for the base period commencing 1 Oct 2001 through 30 Sept 2002 and include two option years, for a quantity of 400 each/8 cases per month a 12 month period. Offerors quoting on products other than MEDRAD must provided a sample and descriptive literature to be considered responsive.
