- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Invitation for Bid (IFB) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-27. IFB 658-60-01 should be referenced on all bids. The North American Industry Classification System (NAICS) code for this procurement is 541620 with a small business size standard of $5 million. The VA Medical Center, 1970 Roanoke Boulevard, Salem Virginia 24153, has a requirement for an ASBESTOS & LEAD PAINT BASE LINE SURVEY. A pre-bid conference/site visit will be held Wednesday, September 12, 2001 at 10:00 AM in Room 213, Building 74. All potential bidders are strongly encouraged to attend. Non attendance at the pre-bid conference/site visit shall not relieve the contractor from performing in strict accordance with the terms and conditions of any resultant contract. A survey for asbestos containing materials (ACM) and lead paint shall be conducted at the locations listed on Attachment A. Attachment A may be obtained by written request faxed to the attention of the Contracting Officer (90C3) at (540) 983-1065. Telephone requests will not be accepted. The asbestos survey shall include items such as pipe and duct insulation, suspect acoustical ceiling tiles/debris on top of ceilings, resilient floor tile, carpet mastic, transite boards, mechanical equipment flexible canvas; or any other suspect ACM found in the rooms. Copies of previous ACM surveys will be available to minimize the amount of bulk sample testing needed for any ACM already known to exist. Current ACM survey will be available for review at pre-bid conference/site visit. A copy will be made available to contractor receiving award. After contract award the Government will supply VAMC drawings from other projects that affect the areas to be surveyed (drawings as needed and selected by surveyors). The lead paint survey shall identify lead-containing paints on all surfaces. A completed Report shall be required furnishing the results of the Survey on a room-by-room basis. The Report shall be prepared such that a person can look up a certain room number in a building, and identify whether any asbestos or lead paint hazards exist in that room. Requirements are complete working drawings showing all sample locations, asbestos and lead locations, and percentage of asbestos and/or lead at each location, and a survey report. A unique sample location numbering system shall be approved prior to starting site survey. Contractor shall provide five (5) complete sets of drawings (drawings shall be 11"x17") and survey report. Contractor shall also submit to the VAMC all drawings and survey report on 3.5" high-density (1.44M) IBM compatible format floppy disks or CD-ROM. The report shall be completed and submitted to the Contracting Officer within thirty (30) calendar days after completion of the survey. All work to be performed for this baseline survey shall meet all Federal, State, and Local codes and regulations; Environmental Protection Agency (EPA); Asbestos Hazard Emergency Response Act (AHERA); Occupational Safety and Health Administration (OSHA); and Housing and Urban Development (HUD). Cost for each line item shall include all costs for survey, testing, drawings, and reporting. Work shall include the services of a certified industrial hygiene consultant with EPA/State certification as an AHERA Building Inspector and Management Planner, as well as a EPA/HUD Lead based Paint Inspector and Risk Assessor. The Contractor is to furnish all labor, material, equipment supplies and transportation necessary to complete all work. Both the asbestos survey and lead paint base line survey shall be completed within one hundred twenty (120) calendar days after contract award. Payment shall be made monthly in arrears for services rendered. Invoices shall be submitted to: Chief, Resources Support Service (04A) VA Medical Center, 1970 Roanoke Boulevard, Salem, VA 24153. LINE ITEM 1; asbestos survey, asbestos bulk sample tests up to 750 tests, five (5) sets of drawings, survey report, and 3.5" high density (1.44m) IBM compatible format floppy disks or CD-ROM containing drawings and survey report. LINE ITEM 2; unit cost per test for additional asbestos bulk sample tests required in excess of 750 tests. LINE ITEM 3; lead paint survey, lead paint sample tests, up to 150 tests, five (5) sets of drawings, survey report, and 3.5" high density (1.44m) IBM compatible format floppy disks or CD-ROM containing drawings and survey report. LINE ITEM 4; unit cost for lead paint tests in excess of 150 tests. LINE ITEM 5; unit cost for furnishing and installing up to 600 labels for pipe insulation after survey, if area has been found to contain ACM greater than one percent (1%) asbestos. Contractor shall supply and install permanent plastic labeling on ACM pipe insulation in each room or at a maximum interval of thirty (30) feet. Labels shall be yellow in color with black lettering to state "ASBESTOS INSULATION". Contractor shall use "Lab Safety" brand #1A-28641 or approved equal. LINE ITEM 6; unit cost for furnishing and installing labels in excess of 600. The Government reserves the right to award all or partial line items. Provisions and clauses found at 52.212-1, 52.212-4 and 52.212-5 apply. The following FAR and VAAR clauses and provisions are added by addendum: 52.228-5 Insurance Work on a Government Installation, 852.237-70 Contractor Responsibilities, 852.270-1 Representatives of Contracting Officers, and 852.270-4 Commercial Advertising. CONTRACTING WITH PARTIES LISTED ON THE OIG LIST OF EXCLUDED INDIVIDUALS/ENTITIES: In accordance with The Health Insurance Portability and Accountability Act (HIPAA) and the Balanced Budget Act (BBA) of 1977, the Office of Inspector General has established a list of parties and entities excluded from Federal health care programs. Specifically, the listed parties and entities may not receive Federal Health Care program payments due to fraud and/or abuse of the Medicare and Medicaid programs. Therefore, all offerors shall review the OIG List of Excluded Individuals/Entities on the OIG web site at www.hhs.gov/oig to ensure that the proposed candidate(s) and/or firm(s) are not listed. Offerors should note that any excluded individual or entity that submits a claim for reimbursement to a Federal health care program, or causes such a claim to be submitted, may be subject to a Civil Monetary Penalty (CMP) of $10,000 for each item or service furnished during a period that the person or entity was excluded and may also be subject to treble damages for the amount claimed for each item or service. CMPs may also be imposed against health care providers and entities that employ or enter into contracts with excluded individuals or entities to provide items or services to Federal program beneficiaries. By signing the offer/bid, the Contractor certifies that he/she has reviewed the OIG List of Excluded Individuals/Entities and that the proposed candidate(s) and/or firm is not listed as of the date the offer/bid was signed. Offerors may find the FAR provisions and clauses on the Internet at www.arnet.gov/far/. Bids are due no later than Wednesday, September 19, 2001, 2:00 PM local time, at which time a public bid opening, will be held in Room 3, Building 16. Bids shall be submitted on company letterhead with original signature of an official authorized to bind the offeror's organization. Bids shall also contain the completed clauses FAR 52.212-3 Offeror Representations and Certifications Commercial Items and VAAR 852.219-70 Veteran Owned Small Business. All bids shall be submitted to Contracting Officer (90C3), VA Medical Center, 1970 Roanoke Boulevard, Salem, Virginia 24153. Sealed bid envelope shall be clearly marked with the IFB Number 658-60-01, "ASBESTOS & LEAD PAINT BASE LINE SURVEY" and the words "IFB DO NOT OPEN".
