Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued. Written quotations will not be issued and each contractor is expected to respond based on requirements. Request for Quotation, RFQ number 672-198-01 is hereby assigned to this procurement and shall be referenced on all correspondence relating to this acquisition. This RFQ is issued using Simplified Acquisition Procedures and incorporates all provisions and clauses as those in effect through Federal Acquisition Circular (FAC) 97-27 and Veterans Affairs Acquisition Regulation (VAAR) 97-1. This procurement is a 100% Small Business Set Aside. North American Industry Classification System (NAICS) 33911 will apply. The San Juan VA Medical Center has a requirement to purchase the following equipment, Brand Name or Equal: Item number one: One (1) each Industrial Acoustics Double Wall Exam / Single Wall Control Audiometric Test Suite. Features 33" noise lock flush mounted cam lift magnetic seal door, tranquil air silent forced ventilation system integrated into SpaSaver ceiling panels, recessed incandescent lights with wall mounted fan and light switch, 24" x 30" window, 10 jack panel, and finished color is desert sand. Room assembly required on site. Rooms are not directly connected but are separated by a 4" air gap bridged by a vibration isolated filler piece. To include Installation Service of Sound Room. Dimensions: Outside complete ( 17' 4" wide x 9' 0" deep x 8' 1" high ). Inside control ( 7' 0" wide x 8' 4" deep x 7' 9" high ). Inside exam ( 7' 4" wide x 7' 0 " deep x 6' 6" high ). Following options may be required: RF Shielding, Fire Rated Panels, Additional Installation for Fire Rated Panels, Oversized Doors, and Additional Duplex Outlets. Required delivery date is 45 days from date of contract award. Inspection and Acceptance shall be at destination. Contractors shall provide the following information: Current Freight On Board (FOB) destination pricing, delivery time, payment terms, Cage Code, Duns/Bradstreet number and Taxpayers I.D. number (TIN). The following provisions and clauses are applicable and are hereby incorporated into the solicitation by reference and will be provided by the Contracting Officer in full text upon request: FAR 52.212-1 (October 2000), Instructions to Offerors Commercial Items applies. Addendum clause 52.215-18, Facsimile proposals also applies. Offerors are required to complete and include a copy of the following provisions with proposals: Far 52.212-3 (May 2001), Offeror representation and certifications Commercial items, FAR 52.212-4 (May 2001), Contract terms and conditions Commercial Items, FAR 52.211-5, addendum New Material, FAR 52.219-6, Notice of Small Business Set Aside, FAR 52.212-5, addendum to FAR 52.212-1, Paragraph (b). Offerors shall provide an express warranty which at a minimum shall be the same warranty terms, including offers of extended warranties offered to the general public. Express warranties shall be included in the contract. The clause at 25.212-5 (May -- 2001), Contract Terms and Conditions required to implement statutes or Executive Orders. Commercial Item applies with the following applicable clauses for paragraph (b): FAR 52.222-26, Equal Opportunity, FAR 52.222-35, Affirmative action for handicapped workers, Employment Reports on Special Disabled Veterans of the Vietnam Era.
