- 17. The VA North Texas Health Care System (VANTHCS) is issuing a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation #549-1-RFQ-161 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This requirement is UNRESTRICTED, and the North American Industry Classification System (NAICS) code is 811219; SIC Code is 7629 for Repair and Preventive Maintenance of Cardiovascular Equipment, and the Size standard of $5.0. A single award will be made. Term of the contract is for a one-year Base Period from date of award through September 30, 2002; with three one-year option periods. Offerors shall include a completed copy of the Provisions at FAR 52.212-3, Offeror Representations and Certifications Commercial Items, along with the firm's DUNS number. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. FAR 52.212-2, Evaluation-Commercial Items is applicable. Paragraph (a), evaluation criteria are as follows: Award will be made to the lowest priced, technically acceptable offer. Technical: 1) Offeror must demonstrate ability to meet the requirements of the Statement of Work and must show evidence of successful completion of service training program for all equipment listed above; 2) Offeror must provide evidence of facility, personnel, testing equipment, parts inventory, licenses & documentation of qualifications of technical personnel. Warranty information shall also be included, and must submit a Quality Control Plan (QCP). Include a narrative explaining the capabilities of your firm to meet the requirements of this solicitation such as financial resources, past experience in similar work, and integrity. Government Past Performance: Include a list of prior government contracts which includes the name and phone number of the contracting officer; name and phone number of your bank and other references. FAR 52.217-5, Evaluation of Options; FAR 52.217-9, Options to Extend the term of the Contract insert (a) "30 days", (c) "5 Years"; and FAR 52.232-19, Availability of Funds for the next Fiscal Year insert September 30, 2001. SOW: Contractor will maintain, check and calibrate according to MFR's procedures and recommended schedule for all the equipment identified above. Within 30 days of award, submit detailed written description of procedures to be performed on each item. Procedures shall be acceptable to JCAHO and Contracting Officer, or designated representative. Services are to be provided Monday Friday, excluding holidays, from 8:00 AM to 5:00 PM. Contractor shall contact the Government within 30 days after award, to arrange mutually agreeable times for inspections. Contractor shall provide, within 30 days after award, a statement describing an acceptable arrangement for notifying the Medical Center when equipment will be made available for servicing. Service will include unlimited emergency repair and call-back repair service and equipment must be restored to proper working order, and is subject to required safety inspections prior to being returned as fit for use. Contractor is responsible for corrective maintenance of equipment when defects are discovered as a result of performance. The VA via telephonic communication shall initiate corrective maintenance and contractor will respond within 2 hours of original call, and provide qualified service representative within 24 hours of original call. Contractor's failure to respond or restore equipment to full performance within 48 hours of original call will result in a 1/30th deduction of the monthly rate for that date, and each 24-hour period equipment is inoperable, up to 2 days. Failure to respond or restore equipment after 2 days will be grounds for Default. If services/parts are required as a result of accident, abuse, misuse, or negligence by other than contractor employees, contractor must report to Contracting Officer, or designated representative, along with estimated repair costs. Contractor shall not proceed with repair due to negligence until notified by the Contracting Officer. The Contractor is responsible for negligence caused by contractor's employees, and is responsible for adhering to OSHA Standard 29 CFR 1910.1030 Occupational Exposure to Bloodborne Pathogens, protection against tuberculosis and proper fire evacuation and safety training. Contractor must provide, for the life of the contract, Proof of Indemnification and Liability Insurance in the amounts of: $1,000,000 per event; $3,000,000 aggregate.
