- Description
- The Department of Veteran Affairs Medical Center, San Francisco, CA intends to solicit and negotiate with only one source under authority FAR 6.302, see Note 22. This announcement constitutes the only solicitation. A written solicitation will not be issued. The following is the salient characertistics of the item under this procurement requirement: Philips MD4 Vascular System, special procedure suite glassware coverage. Glassware coverage to include the supply and installation of allnecessary replacement parts including X-ray tubes, image intensifiers, camera pickup tubes, tetrodes, magnetrons, thyratrons, cryogens {Y or N}, magnets and transducers. Part not included in this procurement are expendable supply items. This system utilizes a 15" HC 3M Image intensifier. The system tube is an SARM 05/11. Camera tube is a Medical Saticon. The services reps must be Philips factory trained and certified. Only genuine Philips parts are acceptable. Response time for service is 30 minutes by phone, 4 hours on site. Access to RTAC (Remote Technical Assistance Center), to resolve up to 50% of system problems before sending service rep to site. Uptime guarantee is 98%. Suite located at the VA Medical Center San Francisco. Mfr. Philips. This is a notice of intent to issue a sole source contract to Philips, Shelton, CT. for procurement of maintenance services for this system. Responsible sources who are interested in this procurment are rquire to submit in response clear and convincing information that identifies their capabilities. In addition, the offeror must substantiate their offer with technical data, training certificates, brochures, pricing list or catalog, terms and conditions, delivery schedule etc. If this commodity is available under GSA contract, please provide the GSA schedule. All responses shall include the DUNS number, TAX ID Code number and Fax number. In addition, the contractor is required to register with the Department of Contractor Registry (CCR) in accordance with DFARS 252.204-7004. As a result of analyzing responses to this synopsis of intent, competed with resulting award under the Simplified Acqusition Procedures. Responsible firms responding to this synopsis will be considered if the action is competed. This is a one-time procurement. The Government does not anticipate any future procurements for same or similar items.*****
