Description
The Department of Veterans Affairs, John D. Dingell Medcal Center is seeking request for quotes to provide necessary supplies/service for normal maintenance to the Hemodialysis Water System. Contractor shall provide exchange service on carbon tanks, deionization tanks and RO membranes. Provide necessary supplies for normal maintenance; salt test kits, filters. Provide two system sanitizations to be performed at the request of Biomedical Engineering as well as two AAMI water analysis for the Hemodialysis Unit. The following sites will receive exchange of tanks at normally scheduled intervals; LL319 Glassware Washer 0115-0501, B4305 Glasswate Washer 0115-0502, LL558 Tunnel Washer 0115-0501, C4420 Chemistry 0115-0504, C4499 Glassware Washer 0115-0507, C1541 Sickle Cell 0115-0507. All responses must include a list of replacement parts, description, quantity, service frequency and cost for maintenance. System cleaning must be performed on Saturdays only. Contractor will submit written report to the Biomedical Engineering Department within a reasonable time frame (to be determined by the Biomedical Engineering Supervisor). Contractor will be required to perform maintenance within 30 days of notification from Biomedical Engineering. This is a combined synopsis/solicitation for a commerical item, prepared in accordance with the subparts in Federal Acquisition Regulation 12.6 as supplemented with additional information included in this notice. This solicotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13. The North American Industry Code is 811219, Medical and surgical equipment repair and maintenance services. Proposed rates should include all costs associated with providing the above mentioned service. The accurate performance of these duties not only have an impact on the quality of paient care, but also the effectiveness and efficiency with which care can be provided. Contractor will be required to attend Safety Training as required by the Safety Office of the VA Medical Center. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation Number Request 553-91-01 is assigned and shall be referenced on any proposals. Partial proposals shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Offerors must also submit with their bids written evidence of training and proficiency (statement of competency) in maintaining any equipment on which they are working. Proposals shall be submitted to the attention of Marilyn Dogan (001R-A). The government shall not be held responsible for quality of fax transmittals. The solication number (RFQ Number ) shall be listed on the outside of your response for identification. Contract term is for one year, plus four option years.
